Does bleeding ever stop
« on: June 19, 2023, 04:52:06 PM »

I have been on Sandrena 1mg and utrogestan 100mg for 4 month and bleeding not stopping. Take tablet every night at 9 and gel just before bed but forgot gel one night. Used it the next morning but bleeding is even worse now. I’m at my wits end. Haven’t bled for years before this. Any advice very welcome.
Should I increase the utrogestan to 200mg. Dr said that might happen after 6 month but other dr said HRT  isn’t for me!!!!

Hi
Utrogestan isn’t great at controlling bleeding unfortunately.

Maybe insert the utrogestan vaginally, to see if that helps with bleeding issues?

Alternatively try a different progesterone or 200 utrogestan?

It’s all trial and error unfortunately.

As above, try vaginally first as more gets to where it's needed. I ceasing to 200mg is a k own strategy for problem bleeding if this doesn't work or you can't tolerate it a mirena is usually the most effective way to stop it. Might be worth asking for a scan to make sure there isn't an obvious cause such as fibroids or polyps and to check the lining isn't building up.

Hello Janey and welcome to the forum. Sorry to hear your Dr said HRT is not for you. You have only tried one type and there are still many others you could try.

Thanks for replies 😊. I might sound a bit stupid here but do you mean the utrogestan that I take orally but change to vaginally 🤔. I’ve just had checks so know nothing else wrong down below re lining etc all good. Marina is not an option as I was I’ll after I got it and never again as it is so painful.

Hi Is anyone on Estalis Continuous 50/140 patches. I've been on them 6 months now and still having irregular bleeds. I've read that its normal to have bleeding up to 6 months. Is this correct? I've called several pharmacists who said 6 months is too long for bleeds and to see your doctor. They said it could be not enough progesterone which controls bleeds. Now I do not know if I should wait bit longer or have my medication twigged.

Can anyone shed some light on this pls.

It takes ages to see a GP that specialises in Menopause.

Now how does a doctor know how much to increase your dose without it being a risk.

Thank you ladies.

Hi Janey21
I'm just wondering if you are not yet post menopause and should still be on sequential? I find that using continuous utrogestan I bleed continually, whereas I don't bleed at all on sequential. Age isn't necessarily clear cut differentiator on whether you are or aren't still in perimenopause by the way, I'm almost 60!

Hi

If you have been bleeding or over 6 months on hrt then you doctor should refer you to have a pelvic Ultrasound scan to rule anything out.

Thats exactly what happened to me on hrt continuous patches, bled up to 6 months. Now my doc is sending me for a pelvic ultrasound. I'm nervous and then I need to get a pap smear test as well.

Has your doctor recommended this to you?

Yes Janey, just use the same utrogestan round 100mg capsules but insert them to the top half of your vagina instead of taking orally. Because Utrogestan is already in the body-identical form that doesn't need to be digested and filtered through the liver before the body uses it, it can be absorbed into the bloodstream from the upper vagina and used immediately, locally. I believe the same can be said for rectal admin, but without an applicator, I've not tried it that way, lol.
I hated the mirena too btw but when I asked about utrogestan vaginally, my GP told me the only local progesterone allowed in the UK is the mirena coil. Yet online there are dozens of doctors who say utrogestan is good to use locally as an alternative. There are far too many "experts" and not enough research done to support or refute their theories.

Hi, thanks for all the suggestions but I’m now on everol conti patches . Easier to use but still bleeding. I will give them a chance. Dr wouldn’t increase dose of utrogestan as they say it’s not licensed 😱 for 200mg. Only got appointment because I said I was just going to increase myself😊. I’ve had scans, biopsies etc all done during covid so I know all ok . Fingers crossed bleeding stops. 😊🤞🤞🤞

I would ask for a referral to a meno clinic, there's usually a long waiting list. It's possible evorel conti will stop the bleeding but there's a good chance it won't. Using 200mg utro daily is a known strategy to stop bleeding so a meno specialist can prescribe it if your gp won't.  Utro isn't licensed for vaginal use for hrt in the UK though it is in the rest of Europe so your gp may be reluctant to prescribe it this way. 200mg vaginally is licensed for ivf but not for hrt. How long ago were your scans? If it's over a year it may be gut be worth asking again in case anything has changed.

Hi Janey

How long have you been bleeding for on these patches cos I'm on Estalis Conti 50/140 and bleed for 6 months. Just had my pelvic ultrasound etc. My doc said if I continue to bleed they will up the dose of progsterone which is one dose up my current.
