Is Mirena my only option?
« on: March 20, 2023, 06:25:29 AM »

Quick history - diagnosed mild endometrial hyperplasia no atypia in Jan 22. Last biopsy Oct 22 no EH but disordered proliferative endometrium.  Bleeding stopped pre second biopsy and now returned to monthly with no treatment. Advised only option is mirena to protect womb if I want oestrogen. Oestrogen levels 160 so manageable symptoms at present but would prefer less feeling like I'm 80! Mood is not an issue and the anxiety that I've had on and off with periods over the years is much better than when younger

Meno consultant advised to protect my pre cancerous cells mirena is the best option and urogestan isn't enough and will increase risk of breast cancer. Its the last bit I'm confused about. I'm hesitant with coil from past experience of pain and moods which meant removal after 3 weeks. Any thoughts?

The medics do love to push the mirena! I’m sure cost is a factor (it’s cheap around £80). If you’ve had problems with it previously I would avoid, it’s your body and your can say no. Some women are fine with it but I had to have mine removed after 6 months because of effect on mood which never got any better - within 48 hours it resolved.

Re utrogestan increasing risk of breast cancer that’s poppycock, utro is a breast friendly option. There’s more risk from synthetic prog.

Sorry I can’t offer any more solutions on your predicament but hopefully some of the others will be along soon with suggestions x

Thank you thats really helpful. Its the mood thing that concerns me plus I get horrible migraines just before my period. Not sure if that's the progesterone

Hi
The mirena can be excellent for controlling bleeding and for many women, it works well.

Only a small amount of the hormone is released into the womb and side effects are few.

You can always have it removed if it doesn’t suit. Nothing is forever.

Best of luck! 🤞

Thanks! I'm not having abnormal bleeding now its all settled. I've been advised I would have to let things settle fior 3 to 6 months before they will remove and that bleeding, weight gain and mood swings are very common? . Would give it a go with a proviso of removal before if it doesn't suit I guess!

It does take a while for the mirena to settle, that is true.

I’ve had no bleeding, weight gain or mood issues with it. You do have to balance it out with a decent oestrogen dose though.

It can be removed if things are unbearable though

Do they think it's the action of the coil that will protect it or the progestin? If it's the former there are other coils that contain less progestin that might suit you better. Kylena is one, I think there's another one that I can't remember the name of. More expensive than morena so the nhs may be reluctant to use it.

The progestin. I asked about urogestan as a pessary and they said not enough progesterone. But more than the nothing I've used and its resolved! Though I can try it but have to take the increased breast cancer risk. Totally confused

The other coil with less prog is the jaydess. My specialist agreed in principle to me having the jaydess but I couldn’t get an nhs practitioner to fit it - they didn’t want to leave themselves open I guess x

Thank you. It seems there's very little option!

Ask him for the research that backs up his assertion that utro is a BC risk? I'm not aware of any and it's considered by some to be the gold standard as it's body identical. If 100 (conti) isn't enough it's a known strategy to use 200 though usually for bleeding problems.

I had to have a mirena because Utrogestan never seemed to work on my endometrial lining and they could never completely rule out atypical hyperplasia, although they could never completely rule it in.

It's a frustrating situation but the Mirena did work much better for me than the utrogestan in stopping the bleeding. Minimal side effects.

Thank you all. I decided to give it a whirl. Hopefully the pain and bleeding settle and then I guess I'll just have to see what happens.

Grheliz1 it's very frustrating! The bleeding resolved on its own. Now I'm bleeding more than I have in a long while. Hopefully it settles.

Hello … can i just ask a question ?
Is hysterectomy not an option . ??

Im due to have a biopsy in the next 2 weeks as my lining is 8mm and the consultant is pushing me to have a mirena . I dont want one to be honest . Really worried about side effects.
But she did say that if the biopsy wasn't normal that hysterectomy may be an option .   Im guessing  there are different levels of what is not normal . Im really confused as she was very anti hrt and mentioned breast cancer and clot risk . Im 58 and thought i could be on it a while longer yet as i feel 108 off it .
