Mirena or utrogestan

Hi all,

I've been struggling with 3 week long periods for 6 months starting off light for a week then heavy. Only really get a week off but in that week I get painful ovulation(?!) with spots of blood. I've had an ultrasound which came back normal and I just take multivitamins and probiotics. This week in struggling with lower abdo pain, bloating and discomfort not sure if it's a uti, bv or thrush. I'm booked in for the mirena next week which in dreading never had it before but my gp said it's the only thing that will help with heavy periods? I originally wanted to try oestrogel and Utrogestran but she said utrogestan won't help. She'll prescribe oestrogel alongside the mirena. I have a tilted uterus which causes big problems during a smear and an ultrasound and I've never had vaginal births.
Any advice please on whether utrogestan works to calm periods? As I'd prefer that option! Thanks 😊

Sorry to hear about your heavy bleeding. This is very common when approaching peri-menopause/menopause.

A question - how old are you?

The Mirena is very effective in limiting bleeding like this whereas Utrogestan is best taken as part of HRT. If you haven't yet reached perimenopause ie haven't started skipping cycles yet then maybe you don't yet need oestrogen - though perhpas you've started experiencing flushes and sweats?

Another alternative if you are still young is take one of the combined pills that suppress the cycle, control bleeding, but give you back some oestrogen which is the same as in HRT. These tablets can be prescribed by meno specialists for younger women in your position. These tablets are QLAIRA and ZOELY.

If you don't yet need oestrogen then utrogestan on its own could be taken as a progestogen to regulate periods especially if you are having anovulatory cycles (no ovulation) - and would be taken during the second half of  the month. You would add in oestrogen when you started to experience flushes etc indicating that your oestrogen levels are dropping - but a Mirena would probably be more effective from what I understand?

Thanks for your reply, very informative! Sorry I should have put I'm 46, nearly 47 have the usual perimenopause symptoms, achy joint, hot flashes, fatigue, anger all of a sudden but then I can have a good week. My oestrogen level has dropped from 400 in 2018 to 200 this year. I used a fsh urine test and the second line appeared instantly too.

I've not heard of those tablets you mentioned, thanks I'll look them up!!
