HRT after Oophorectomy
« on: July 24, 2022, 09:06:18 PM »

Hi, just wondering if any of you lovely ladies are managing your menopause symptoms without HRT post oophorectomy please?

I am really struggling with progesterone (utrogestan) and so am seriously considering coming off HRT altogether, can’t lie, I am a little apprehensive though :o x

Morning.  Was this not discussed prior to Surgery?

Many don't get on with progesterone  :-\.  Hopefully someone will be along ......... you could get onto a waiting list for a dedicated menopause clinic - not a gynae - or send an e-mail Dr Currie on here.

Morning CLKD thanks for your reply!

No unfortunately not, it’s been 8 years since my surgery and think I’ve tried every type of HRT in that time!

I had a private consultation in April last year and following that was prescribed utrogestan vaginally along with estrogel and I have been using Tostran gel for the past 7 months too.

I am scared to come off HRT but can see no other alternative apart from maybe continuing with the estrogel and having regular scans privately and I have no idea how I would go about this  :(

  has the progesterone become a problem recently?

Any change can upset the body and if you are generally well, you don't want that risk?

I would ask your GP in the 1st instance if they support an oestrogen only regime with occasional progesterone and scans.  As well as getting onto a Clinic waiting list for dedicated advice.

In the meantime, put your regime into the search box on the Forum to see what pops up?  Each drug separately probably has threads already.

Hi,

I have my oopherectomy 5 years ago and have had times where, like you I think of going without. And nothing was discussed with me prior to removing them either. Just a glib comment of you'll need hrt. Nothing about how difficult it can be to get a balance. I keep taking it based on not wanting to lose the benefits of bone density etc. So I persevere based on that worry. Im not on testosterone but my specialist is against it (gynaecologist) and Im a bit frightened of going on it - my ovaries were removed because of tumours.  But lots of what I've read about oestrogen are very much for taking it after ovary removal especially if under 51. Im also a believer in progesterone not just being for the lining(just maybe not at such high does) - but that's not prescribed that way.  I think for us with no ovaries we need a balance of both hormones and its getting that right. Its like at some points one of the two hormones is too high or too low and hence feeling crap. Sorry not much help only to say I get how your feeling, sometimes you just want to give up trying to feel normal again1

MayB you both need to discuss with a dedicated Menopause Specialist?  It is negligent that Surgeons don't discuss the options  >:( and how important HRT will be in future!  This should be included in any future training!  Again, women have not been supported when offered major surgical intervention.  :'(

Yes I agree, I had no idea how it would be and it was definitely a shock. I don't feel like I got any support around it x

So Im on 125mg patch. From my levels it gives me around 400pmol absorbed. Also Utrogestan cyclically. Im lucky it doesn't affect me too bad. But I think I need a small amount of progesterone daily just not the 200mg. Ive thought about compounding, even though I know the BMS doesn't advise(that's a whole other discussion! 😂).
Im not on testosterone, even though It may be the missing link I need. Given my tumour history my specialist is hesitant. I am too. I wish we had the studies so I could make an informed decision!
If I had gone through natural menopause I don't know if I would have taken HRT - or if I will continue when I get to 51- Im now 43. I have just had a bone scan and I have a feeling it will be low so I feel, for me, even though I don't feel great, that I will continue with the HRT till 51.
I think people presume with no hormones added by their own body that dosing is easier but its certify not for me. I just feel Ive aged 20 years since my op.

Do you take it vaginally or orally? You can take less if used vaginally as more of it gets to your womb (according to Newson). Conti or sequi? Some women who are meno use a long cycle (6 weeks) if they don't get on well with utro, that's what I do. Perhaps try 100mg vaginally on a 6 week cycle and have a scan in 6 months to check it's OK?
I assume you know it's important for bone health you take it at least until your early 50s if you possibly can.

Thank you, I really don’t know what to do for the best…I’ve not been taking anything for the last 4 days and already the insomnia is back…maybe it’s all in my head though?! 🤷‍♀️

Nope, certainly not in your head!   Although hormones can impair our sleep patterns as well as moods ....... - rages!
