The point about the early 12 month cut off point before any bleeding should be investigated is so that any potential problems eg endometria hyperplasia or abnormal cells which could lead to cancers would be picked up early. The younger you are (especially below the menopausal average age of 51/52), the more likely it is that you may well get random ovulations after cessation of bleeding much later than this. This is recognised in the guidance that under 50 contraception should be used for 2 years following last period but over 50 for 1 year.
Most investigations will turn out to be just this, ie random late ovulations but always better to be investigated than not.
