New girl and hoping for advice

Hi ladies.

I'm 43 and have been in a chemically induced menopause (using a Gnrh) since November 2016 due to significant and crippling PMDD. I was on Zoladex until July and I'm now on Prostrap. I started with add back HRT in March.

I've changed HRT a couple of times and I'm now on Femiston Conti. My gynaecologist is really difficult to communicate with and every time I try to explain the way I'm feeling she tells me I can't possibly be feeling a particular way as my ovaries are shut down. She has told me there's no link between mood and menopause.

I feel the Gnrh wears off and I'm certain I'm still ovulating. I'm ordering an ovulation kit to check this. I have heightened anxiety, low mood and irritability at a certain point each month.

On top of this I feel in general pretty miserable, I feel bloated and generally down and anxious. I'm wondering if the HRT isn't working well with Prostrap? Or is this what the menopause feels like?

I have zero sex drive, however I also have marital problems so unsure if this is menopausal or life! My confidence in my appearance is zero, I feel like I've lost myself.

Apologies for such a miserable introduction. I feel I can't talk to my gynaecologist and feel lost.

Can anyone offer any advice or suggestions?

Hi and welcome to MM

You are not alone. Your gynae is quite wrong, low mood and anxiety are definitely menopausal symptoms. It you have problems your partner this will be adding to the stress so I suspect you are struggling to cope on many levels.
As you are on a continuous HRT type, you are using progesterone all the time and this can effect mood - your gynae should know this and in fact pmt symptoms are common with any progesterone.
I believe there are various ways of treating pmdd but the treatment you are receiving is pretty standard, however, you may do better on a different HRT type. Femoston does suit many women very well ( it does have one if the best progesterones)  but you might do better on a different type.
Do look at Professor Studd's website as he is the expert on treating things like  pmdd and any hormonal problems.
Keep posting - we may not be doctors but many of us have been through quite a bit because of meno problems, so we're here to help if we can. DG x

Thanks ever so much for your reply. I hate it when my gynaecologist appointments come around as she's so dismissive. Even when I'm telling her I had tender breasts, she said it wasn't possible. I leave feeling like I'm neurotic and perhaps a bit mad!

I'll take a look at the website thank you. Femiston Conti suited me better on a different Gnrh, bizarre!

 My gynaecologist won't entertain the idea of a hysterectomy with ovaries removal as she says I can stay in this medication long term. I know other ladies on a PMDD group I'm part of have had a different experience altogether. Treatment and advice seem to differ so much depending on your gynae.

Thanks again. At least now I know there are emotional changes with the menopause and it's not just in my head.

emmajjenkins - I think you need to pop back to your GP adn ask to referred to a different gynaecologist.  Explain that she doesn't give you the support you need and, having done some research on reputable site such as MM, you feel you need a second opinion on the treatment you are receiving. If your current treatment is not working well for you, then it needs changing - your gynaecologist should not be dismissing your symptoms nor should she in any way make you feel belittled. I bet she is still quite young - let's hope she gets a tough menopause when it's her time to go through it.

Quality of life is very important.  Sadly there is still not enough understanding of how hormones can mess with you body and mind if they are out of balance OR, in the case of menopause, the simple lack of hormones.

Write down all your concerns, symptoms, and questions - - I find at any doctors appointment this can help prompt me when I'm feeling rushed or patronised by the doctor.  This evidence can also go into your file so if, in the future, a doctor claims you didn't raise an issue it's there in black and white. It also forces them to address your concerns.

Sex drive might be improved with some testosterone.  Maybe you could be given Tibolone (Livial) instead fo Femoston, as this can be very effective as hormone replacement and also help libido - could be well worth asking about? 

Keep a diary to show your fluctuations in mood etc.  Note things like breast discomfort, poor sleep, aches, pains or anything that you feel is relevant. PMDD is nasty, it can and should be treated appropriately but we are all different so no one treatment will work for every women, so some understanding and flexibility is needed from you specialist. Believe it or not, she's probably in her 50's!
