Utrogestan-no bleed

Hi, I've been on Utro for about 3 years now .

I have a 50 mcg patch for 10 weeks followed by 200 mg Utro for 12 nights.
Everything has been brilliant. :)

However,for the first time I have not had a bleed after finishing the Utro
( I normally have a light/normal bleed 2 days after finishing Utro).

Do you think there is a problem or has anyone else experienced this?

I am 59 and post meno...( diagnosed early at 38)

Hi Diana, I too am on a 50 patch followed by Utrogestan, but I take mine day 15-26 on a 28 day cycle. I have only completed one full cycle and didn't bleed at all. This happened to coincide with a visit to the doctors to get the result of blood tests, and she said that my natural hormones had given up the fight! and that was how she explained the lack of bleed. I will be interested to see if I bleed at the end of this second cycle. She didn't increase my patch though, but I think I really do need more something because it is not controlling my symptoms. Maybe your own estrogen levels have dropped too. Hopefully someone more knowledgeable will be along with an explanation. x

Thankyou for replying.

Yes,I did wonder if I need a bit more estrogen.Although the majority of my
symptoms are under control,I put up with a few minor ailments.I might
try the gel as it seems easier to regulate the doses.Utrogestan is
a miracle progesterone!(compared to the other choices)

Hi Diana and Grumpymum,

I'm on 25mcg Estradot and 10mg MPA for 12 days and last month, for the first time ever, I didn't have a bleed. I have no signs of my own hormones lurking around this month either so I guess I will miss a bleed again.

Just to say, I had an ablation a few years ago for excessive bleeding... it worked. A scan of my lining last year showed that it was almost non-existent so maybe this and no hormones except HRT explains it all.

I did read in the menus on this site, that 85% of women get a withdrawal bleed on cyclical HRT - 15% don't. Maybe we are in the 15% category.

Wouldn't that be perfect...no bleeds..
Think I will try bi-monthly for now and see what happens.

Do you get on Ok with the MPA? I had big problems with it.

Hi Diana

I must say that it is surprising not to get a bleed when you are on a 3 month cycle, but not at all unusual in post-menopause on a conventional 4 week cycle. I have also been on utrogestan for 3 and a half years, always on a long cycle - but never a 12 week one like you are. Mine are 5 - 8 weeks  ( I started two monthly and I may go back to these). I always get a bleed - enough to call a bleed.

Are you taking utrogestan orally or vaginally? I was going to suggest you might not be absorbing it if taking it orally - even so if the lining has built up then you would expect some sort of bleed even if the dose wasn't quite enough. How long is it since you stopped the progesterone ie when was your last dose?

I wouldn't have thought your oestrogen levels would be changing at your age - I would give it another cycle and see what happens and then maybe mention to a doc (your specialist) or earlier if you are worried. However lack of bleeding isn't usually an issue - it is usually unexplained post-menopausal bleeding that usually is investigated promptly as it can be a signe of endometrial hyperplasia and cancer.

You are wise to question it - I think we all need to be aware of what is normal or not as we continue to take HRT into our 60's (I'm 62).

Thankyou for your reply.

I use 50mcg patch for 10 weeks then Utro for 12 nights orally.
My last Utro was last Friday and as regular as clockwork I always had
a bleed on the Sunday (2 days later).Bit surprised not to get anything
this time,although bleeds are lighter.

I wondered if I might need to up my patch a bit although when I tried
75 mcg it was a bit too much.I tried cutting off a bit but they didn't stick
that well.

I've got a Doctor's appointment next week just to make sure all OK. I am
going to ask if I can change to an estrogen pump.I think it might be
easier to regulate the dose.I tried the estradot that you are on but I had
a skin reaction .

Do you know what the doses in the pump are compared to the strength of
patches?
