donnacrichton - the Estradot patches come in quite a few different strengths (low, medium and high) but the strengths are in 'mcg' not 'mg'. You cannot compare the amount of oestrogen in the pills to the amount of oestrogen in the patches because the delivery is very different. If you look at 'HRT Preparations' under the green headings to the left of this screen and then click on Estrogens you will see all the differents types and strengths of oestrogen.

The diary is not necessarily for the doctors benefit, it is for you to look back at how you felt over the weeks on certain treatments so you can tell the doctors exactly how you were at a particularly stage.
If you have an appointment with the gynae in August I think your best course of action right now is to stick with what you are taking at the moment - either 3mg or 2mg in pill form -
don't keep switching back and forth - give your body a chance to adjust and for you to see how the hormones are effecting you.
Before you go for the appointment write out all the symptoms you are getting and all the questions you want to ask.
I think you have got yourself so stressed out with all this you are finding it difficult to work out what is for the best. Tell us a bit about your life, do you have children to look after, do you have a stressful job etc?
If you are desperate(you do sound very stressed), then you really ought to see your GP a.s.a.p. as sometimes they can be more objective about you as a person rather than a specialist who is just looking at one aspect. Your GP could possibly prescribe a different patch or the Oestrogel for you to try.
