I felt awful. Could it really be just that? I shook, light headed, hot, cold.

Yep.  Kind of a blood sugar 'drop' in that the body suddenly requires an energy boost with nowt to draw upon.  Hormones can certainly cause this ..... which is why, in the 1990s, The National Association for Pre-menstrual Syndrome advised me to eat every 3 hours, 24/7.  Even in the night!  It's a habit that I try to stick with these days so have snacks to hand when travelling.  Or walking we have fruits/nuts and Dextrose tablets.  Some like Kendal Mint Cake  :-X  ;D which went to the top of Everest and back in 1952. 

When working my worst time was around 3.30-4.00 p.m. so I had a banana or nuts B4 driving home.

If thats what it is, it caused really high anxiety. Did you get that? Panic attacks.

I've had panic attacks since the age of 3.  [long story short].  High levels of anxiety prior to events; visiting family; holidays.

Saw the gp again today.
No leukocytes in my urine but still blood (although I've always had a trace)
Got 5 days of a different antibiotic as I'm feeling so poorly. More blood tests this week.
