Cyclic progesterone question.
« on: June 23, 2024, 12:53:46 PM »

👋 hello
I am On cyclic progesterone day 25 to 26..
Day 8 off progesterone and no period...
What would you ladies do...how do I count day 15 again? Is day 1 the first day off progesterone? In  the
past it would take me about 6 days to start a bleed...does it mean my estrogen is too low? I did a blood test 2 days after stopping progesterone and my estrogen was 200...I am going up to 75mcg as still have some symptoms...
Thank you!

It doesn't have to be taken cyclically, you might be at the point where you can just take it continuously and remain bleed free 😁.

If you are keen on taking it cyclically, it is typically just two weeks on, two weeks off.

If you are on a reduced progesterone regimen generally under the direction of a specialist, counting the first of the month as day 1 can work, if you no longer have a regular cycle of your own to go by.

Either there hasn't been enough build up or your own cycle is overriding the hrt one and you'll bleed when your own cycle decides. Medical advice is to to carry on with the normal hrt schedule but if it's consistently out of step you may want to adjust it. Not everyone has a monthly bleed so you should go on symptoms to increase oestrogen not the lack of a bleed. 200 is low though so unsurprising you still have symptoms. Presume you mean days 15-26 not 25-26? And if it's 25-26 I want that regime too  :).

Oh yes day 15 to 26 ;D..
I actually thought my period would come sooner as I normally feel.the "ovulation " and felt it on day 10 of my cycle...but nothing...
Thanks so much for your response if nothing happens I will carry on as if it did. Annoying part is I had all the symptoms of it coming -pain , bloating, irritability...
Is there a guidline of what your level should be on a certain dose; ie if I don't produce much estrogen should my level be higher than 200 on a 50 patch? Just wandering about absorbing it.

Thanks so much!

Thank you! I still have a fairly regular cycle...I started hrt just a few months ago and so my doctor wants to keep it cyclical for now. I probably would like the continues better as I like the progesterone days as it calms me down and helps to sleep.

Taking it cyclically is not medically necessary, it is traditionally prescribed that way with the intention of avoiding irregular bleeding in those who still have endogenous ovarian activity.

However a) this is not always what happens and some women get regular bleeding, some get none and others get anything in between and
b) there is no safety concern regarding taking it continuously, in fact endometrial protection is superior with continuous use.

If you are getting benefit from it for other issues such as sleep and don't mind the fact that you MIGHT get some irregular bleeding, which may be a nuisance but is not harmful in any way, than absolutely it can be taken continuously.

Prescribing guidelines usually recommend double the dose if being taken cyclically eg 2 x 100mg micronised progesterone nightly for 2 weeks out of 4 or 1 x 100mg every night.

So your prescription SHOULD have the same overall quantity of micronised progesterone on it whether you take it cyclically or continuous.
