Hello and question about shaking/quivering mid-afternoon

Hello, and thanks for accepting me to this forum. I have looked through it many times and it is so helpful.

A little about me - 53, 4 months with no periods, long covid (hellish), on HRT. No other health concerns, eg diabetes, heart issues, etc.

My question - I have experienced fairly violent internal shaking and tremors for the past 3 years, but HRT brought it under control. It feels like all my internal organs have turned to jelly. Since my periods stopped, 4 months ago, I have this horrendous dip mid-afternoon, always at 2.45pm, give or take a minute, where I am consumed with feelings of dread, I am freezing cold, and shake internally. It is absolutely horrid and I have learned to never make any decisions, or take seriously any thoughts of gloom I have, during that time. It always lifts around 4pm.

Due to the timing, I am convinced it is a hormonal fluctuation. I don't have blood sugar issues, though I do think it might be cortisol-related.

Has anyone else changed anything with their HRT to try and combat this? Do you think I just need to increase it a bit?

Just interested to know if anyone else has had this kind of very specific symptom, and if they have dealt with it.

If it's the same time it could be a sudden energy dip.  Although I've never had internal wobbles, I would get strange nausea around 4.15 so always had a banana B4 I drove 7 miles home at 4.45 p.m.

How is your diet overall? 

Which HRT regime R U prescribed?

Thanks CLKD for your reply. To answer your questions, my diet is very good so I don't think it is that. I also eat regularly to ensure my blood sugar remains steady, and don't really touch sugary food that would cause a spike/drop. I think the issue is hormonal, and certainly feels that way.

Currently on 2.5 pumps oestrogel (2 night, 0.5 in morning). I don't cope well with big changes in oestrogen, so any changes need to be done slowly. Utrogestan continuously; 0.3mg testogel and vagifem.

MayB put the product names into the search box : individually :  and read the threads that pop up.  Make notes ;-). It mayB that a small increase will help.

I used to get those feelings, mine would start just out of the blue, anytime.
I think it’s hormonal anxiety.
One day, I was talking to someone at the park walking my dog, and I just started shaking, I felt awful, and this feeling of dread came over me. It was truly terrifying.
I was glad to get home.
You have my sympathy, but I do think it’s all hormones.

Forgot to say, I don’t get it anymore, I still suffer anxiety sometimes, but those trembling and shaky feelings have gone, I changed my hrt in January this year, after finding out I wasn’t absorbing the prog in the tablets.
I’m on gel and utro now, and hopefully, 🤞🏼 I’ve found my balance.
You could try and up your hrt, it’s all trial and error, I’ve found the higher I go, the more anxiety I get. But I am 9 years post meno now. 🤷🏼‍♀️

Thanks for this jillydoll. I had a blood test done and my oestrogen level has dropped by around half since my last test on the same dose of oestrogel. That shows me it is hormonal. So I have made an appointment to see my menopause specialist earlier than planned. I will chat with her and I am sure she will increase the oestrogel.

I really don't want to be going higher than 4 pumps because my GP won't prescribe it. She currently does as the menopause specialist asks, and I get all of my HRT on the NHS except for testogel.

Anyway, my wellbeing is more important than worrying about what my GP will or won't do!

Yes - 4 pumps is the max she will go to because this is what the formulary states. She also won't prescribe testogel (or any testosterone) since not on the area formulary for women. She will not go off piste, and that's her call to make since she is prescribing.

I will see what the specialist says, and I can always pay privately if I need more oestrogel. It is just a bit wearing, having to manage it all. Hopefully 4 pumps will do the trick, though.

I know my meno specialist said she thought I’d need about 3/4 pumps.
After trying that, I knew it was too much, infact at one point I was walking around like a zombie, so tired, no interest in anything really sore heavy boobs, and felt like curling up and dying. I was ready to throw the towel in.
However, after a good think about it, I decided to go lower, and I’ve found just 2 & half pumps IS enough for me.
I think I also struggled with the urtogestion at first, but now, 6 months on, I’ve no trouble at all.
I used to think every night,..oh nooo, here we go again taking this bloody thing, but now, it’s ok. I really don’t mind taking it.
Looking back, I’m glad now I didn’t stop it all, my meno symptoms are quite bad when they get going, and I didn’t really want to go through all that again.

OK, so I saw the menopause specialist and she agreed that the afternoon 'dip' is a hormonal thing. Prescription has been increased to 4 pumps (and I take 200mg utrogestan continuously). Didn't have the dip yesterday so we will see if that continues over the next few weeks.

2.5 pumps was great for me, jillydoll, for so long. Then oestrogel came in those cylindrical bottles and that's when it all went a bit doolally. Might be me progressing deeper into menopause, or maybe I just need more of the new style oestrogel. Whatever, I will see what happens next in the the next exciting instalment of this weird world of menopause.
