I think using ovestin internally is essential for atrophy.

I agree. An alternative such as vagifem is okay too. :)

I don't insert it. I use it externally at night.

Are you inserting any localised oestrogen at all?

Not sure if you're asking me but yes, I use vagifem internally.

I use 'ovestin' internally and smear a little 'ovestin' or KY Jelly in the labial region.  This keeps the vulval area soft and less itchy.  Using 4 me when necessary.  The idea is to plump up the vaginal tissues to ease soreness, light bleeding and the repeated urge to pee - VA mimics urine infection-type symptoms really, really well  >:(

Some use 'vagifem' internal with 'ovestin' or other mositorisors in the labia.

It's a small amount of active ingredient, designed to treat the effects of vaginal atrophy .
