Hello,

I am new on here so sorry about the long message.

I am 45 and had premature menopause at 20 in my early 30’s I started hrt (elleste) I used this for over 10 years and had my “fake” bleeds every month with no probs.

As I am now in my 40’s and have been on the same hrt for so long and my concerns I had about being on it for so long  my doc suggested I changed to a “safer” more body identical hrt with no bleeds… sandrema gel 1mg and 100mg of Utrogestan… all worked well for over 6 months no bleeds and felt fine. Towards the end of last year I started spotting and bleeding… had ultra sounds all ok. In March this year Doc suggested I increase my gel to 1.5mg and increase my Utrogestan to 200mg for 3 months to control bleeding…. My bleeding the last few weeks have been worse than ever… proper periods every 2 weeks.

Has anyone else had this? I thought increasing to 200mg was supposed to fix it but has made it worse. Feeling really down about it.

I’ve not had exactly this, but I do notice if I increase the oestrogen part of my HRT I have bleeding and spotting. I take 200mg utrogestan continually, and reducing the oestrogen stops the bleeding.

Hi SLUC,

I had bleeding after being ‘bleed free’ on a 75 mcg patch for over a year, ( 75 mcgs is the equivalent of the estradiol dose you have been asked to increase to now) whilst taking 100 mg utrogestan. When you had your ultrasound, did they advise how thick your lining was? I had both a scan and then a hysteroscopy because they detected fibroids on the scan, but when I had the hysteroscopy, my lining was atrophic ( I had to look it up - it meant very thin :o) and I had no fibroids. I too was asked to take 200 mg utrogestan daily, but I could t tolerate it unfortunately, but I also still bled. In my cases I don’t absorb eostregen very well and that could be the reason for my very thin lining. I think medics automatically think that we all have high Estrogen levels. If you haven’t done so, you could ask for your estradiol levels to be tested? You would know then if you were absorbing properly.

I had 2 scan’s they advised my lining was 5mm in Sept 22 and 3.8mm in March 23 and I have been advised both times that is fine for me.

I am so scared of this bleeding that I have gone back down for 1mg of gel and 100mg of Utrogestan in the hope it stops!?

Hi SLUC,

It’s such a rollercoaster isn’t it? I’ve had two lots of bleeding as well, both a year apart on the same HRT  ??? I know how frustrating it is. Your lining sounds fine at those measurements, especially the last one. Hospital Staff & NHS will always go by the book in what they advise, but one size doesn’t always fit all, as we’ve both found out. Don’t be scared of the bleeding. I was assured at the scan/hysteroscopy that as it had been investigated, I wouldn’t need another scan unless the bleeding continued past another 10 months….so try and bear that in mind.

It’s a shame you’ve had to revert back to your original dose of gel though. The likelihood is that it is the utrogestan that is causing the bleed. You could try asking your GP to refer you to the menopause clinic ( long wait list though), or you could try a mirena coil. It’s not ‘body identical’ though, but would hopefully stop you bleeding ( it’s pretty guaranteed). I’m on my third mirena and although I didn’t want one for HRT, as I too wanted body identical hormones, it seems to suit me. You seem ok with the non - body identical hormones so it might be worth a try. Progesterone doesn’t bypass the liver, and goes straight to the womb, so it’s regarded as ‘safer’ than the previous synthetic progesterone that you used to take. You could also ask for an estradiol blood test ( just out of curiosity to find out your Estrogen levels).

Thanks for your reply Teresa the 10 months is reassuring to hear. When I had the scan in Sept I also had a hysteriscopy which was fine and then at the scan in March they said I didn’t need another. Seeing lots of blood when I am not supposed to be seeing any just really scares me and I just assume the worst. Very hard to see a Menopause specialist on the NHS so I have a booked a private appointment in the the next couple of weeks to change my HRT as this obviously just doesn’t suit me even through the 1st 6 months were fine!

Well done in booking a private appointment. Unfortunately it’s what we have to resort to it seems. My bleeding used to go on for two weeks so I do understand. It wasn’t ‘spotting’ at all. I hope they sort you out  :)
