early meno - struggling with HRT i think? please help :-(

Hello! I'll try keep this as short as poss. I'm 31 years old, diagnosed as being in the peri-menopause by specialist hormone BMS endocrinologist and my gynaecologist due to consistent low estrogen levels (the HIGHEST reading i've had has been 295 pmol, the majority of the tests said my estrogen was 'undetectable'). I've had every meno symptom under the sun so was glad for the diagnosis finally after years!

I have been started on the Mirena 7 weeks ago and at the same time Evorel 50 patch. After approx 1 week, i felt AMAZING, mood transformed, joints back to normal, etc etc just felt amazing and so glad for the relief! this lasted 6 weeks, then i had a massive dip, all my symptoms back, even ended up with a UTI!! - rang my GP and she upped me to 75 patch.

Now, i know im slightly out of the norm due to my age and the amount of estrogen i'd need probably to keep me level. BUT, i still dont feel great, lethargic, achy, flat mood etc...  :-(

what shall i do? why am i feeling like this now when i felt fantastic to start with? (a complete transformation btw, not just a slight improvement!) I'm scared that my estrogen is now TOO HIGH?! but i dont have any bleeding (havent since i had the Mirena fitted)...

what do you lovely people think? Hope somebody sees this and sheds some light!

Hello and welcome to the forum. Something that stood out to me from your post was you got a UTI. I am wondering if you have vaginal atrophy and need some localised oestrogen for this. This is caused when the area lacks oestrogen and needs treating with a cream or pessary. Just thought I would mention this in case you are not aware of it.

I have read that some women get a high when first starting HRT and then it seems to drop. I am not sure why that is but I would not think this means you are on too much oestrogen. How knowledgeable are the specialists in menopause as often a menopause clinic can be more knowledgeable.

It can take time for your body to adjust to an increase in oestrogen so it could mean waiting a little bit longer to see if you need to increase it.

I am sure you will get replies from members who know much more then me. I just wanted to welcome you and share my thoughts. :)

Hey!
I was peri and started my symptoms at 33 so fully understand the struggles!
I also too get the high when adjusting dose then seem to crash again after a few weeks. Something I would advise would be to stick at each dose for the recommend 3 months, I changed my dose constantly in a desperate attempt to feel better and have caused my body no end of grief!
If you estrogen is low (mine was only 59 p/mol) then it will take some time to build up, unfortunately it’s not unusual for it to take a year or sometimes more to get the HRT right. I still haven’t and I’m nearly 3 years into it, but that’s due to my impatience and changing dose too much. Give it time - I know that’s tough but hopefully you will get there sooner that way. Tortoise and the hare springs to mind!

Hi Flossie, I hope you see my reply as I’m not sure if I’m doing this right! Yes I am on vaginal estrogen but I’m guessing not enough! (I do get tears regularly so I suffer quite badly down below)!

The endocrinologist that diagnosed me it’s called Dr Mukherjee, she actually sits on the advisory board for the British menopause society! Apparently one of the best in the UK! She seems to think I’d need to be on at least 100 patch eventually anyway due to my age but I can’t help when I have a dip feeling desperate and confused I guess!

I’ll give this dose the full 3 months now. Thank you for your comment it means a lot! x

Oh wow, sounds like you are in very good hands then. That is wonderful. As she thinks you will eventually need 100 then it sounds like it is a matter of getting used to the 75 before you can increase again. It is horrid to have the lows though and I am sorry you are battling that.

With using the localised oestrogen I find I need to use it every single day. If I try to reduce it then I get a terrible flare. Perhaps you could try using it daily too until your symptoms improve and then you can decide if you wish to reduce it.

Hi Madge ☺️ it’s horrible isn’t it! Especially being very young it took me a long time to get a diagnosis! Thank you, I’m going to stick to the 3 months of this 75 dose now and see how I get on! Well, most of my readings they couldn’t get a reading because it was lower than the nhs machine would test to, so for alot of mine I don’t actually know the result, I just know it’s low. But then my mid cycle ones did go up slightly to 175 - 295 being the very highest I’ve had over the year of testing. It’s so tough isn’t it, just wish there was a one size fits all solution to this!! She was lovely, very knowledgable, I had to wait 6 months to see her privately at Spire Manchester she’s in very high demand it seems. Yes I will start using it daily once I stop the antibiotics and get rid of the thrush that’s been caused by the antibiotics!!! Treating one thing to then treat another! Driving me crackers! I remember the days I had a normal v*gina!!!

I know the antibiotic, thrush cycle well! You may find it helpful to take a probiotic as antibiotics can kill our good bacteria. I hope the thrush goes soon.
