Utrogestan confusion

Hi there , I'm 50 , on Oestrogel 2 pumps daily currently , along with 200mg Utrogestan for 12 days of the month. I recently spoke to a gp on the phone ( couldn't get an appointment at my surgery ,never can, ridiculous), who has left me completely confused . At the moment my periods( or bleeds, how do i know which they are if not regular ?) are approximately every second month. She told me that this is worrying as the utrogestan should be causing a bleed every month, or not at all every month (?) It shouldn't be erratic if I'm taking utrogestan !! I thought that my periods would tick along in the background , at their normal erratic cycle for my perimenopausal state, regardless of hrt ! ? She said that it is worrying, and I possibly need a scan to ensure the lining is not thickening . She has totally confused me.

I have also been struggling hugely with the utrogestan . Feel very low and extremely anxious when I am on it, although I tried to start using it vaginally the last few nights ( I am concerned I am not pushing them up high enough into my vagina, as have prolapse ( sorry if tmi) . I did notice mood was better on the days I did this though. Then I decided to take orally last night and feel wired and so anxious this morning . I would like to use vaginally but does it matter how "far up" they are ? I don't have utrogestan pessary, if there is such a thing.

My mood is so bad , anxiety, moodiness, depression for most of the month that I'm considering going on anti depressants but am worried about taking them.

Any advice greatly received , particularly some reassurance about the lack of bleeding . This is normal in peri, right, with or without hrt ?

Hi you are right… sometimes bleeding doesn’t occur when using Utrogestan. I think the statistics are 20% of ladies don’t bleed on sequential Utrogestan x

I bleed about 1in 3 when my own hormones don't contribute. How high up can you get the utro? Most people don't use an applicator so it's just as high as you can get it with your fingers.

Hi Shamo - I can’t answer to the bleed question as I’ve had minimal bleeding on utrogestan. The only spotting/period I got was recently because I was experimenting with cutting my oestrogen patches.

Re: vaginal delivery of utrogestan, I switched to vaginal delivery ages ago because oral delivery exacerbated side effects too much. This could be why you report that your mood eased a little when you tried it vaginally. You’d have to take it for longer to fully know. As for how to insert, I just use my finger to push it up as far as possible, usually close as possible to  the cervix. That should be ok, but maybe go for an applicator if you think it might be easier that way :)

Thank you.  I was worried because the gp led me to believe( incidently she's a women's health specialist!) that it may be a worry that I am not bleeding each month. Your emails have reassured me a little. I also use vagifem. Can anyone advise how it works , taking the utrogestan vaginally if it'son a night when I have to use vagifem too ?(2-3 times per wk).
