Post Menopause Bleeding

Morning everyone, I'm after some advice - this is my first post, so be gentle lol :)
I went through an early menopause at 39 - so I'm now 5 years post, at 44.  I've been on HRT for the last 4 years and have found out I've not been getting any progesterone - so that's been remedied as we speak with medication (Utrogestan).  I've also been given Estradot 50mcg twice weekly, Vagifem twice weekly and VM moisturiser for VA.

I was doing quite well until Feb 2021 when I had what I would call a light period.  Bit of bleeding and gone after 3 days.  GP referred to gynae on 2 week wait - I had an internal exam, ultrasound and an MRI (as they suspected I had a cyst).  All clear - so discharged.  Had the same again in Mar & April 2021; so had a hysteroscopy and a biopsy taken.  Again - all clear.  At this point, my HRT was changed from Kliofem to Tibolone.  All was fine for just over a year - then in July 2022 and Sept 2022, I noticed a blood upon wiping and a bit in the toilet.  Went through GP and Gynae once again just before Christmas.  Again - nothing can be found, I couldn't deal with another biopsy in clinic - but she wasn't concerned.  However, as I've had quite a few 'bleeds' and nothing found.  Where to next?  Gynae have suggested going back to GP to change HRT BUT have listed me for a laparoscopic hysterectomy (keeping ovaries); as it could be a couple of years before I receive an appt.  Gynae wouldn't list me for a Mirena and the hysterectomy - so I went with that.

I've since been to a Meno Specialist who has sorted my HRT out and I'm on a 3 month test now.  This lady has listed me for a Mirena and I've an appointment to get one fitted on the 31st of this month.  I'm after some advice and guidance from you lovely ladies as to whether I've been advised correctly and if you have opinions one way or the other - in relation to the Mirena OR hysterectomy.

Hi welcome to the forum 😊.

The Mirena is a very good option for providing progesterone for hrt. I would certainly try that before opting for a hysterectomy. x

I agree with the above. Every surgical procedure carries a risk and personally I would only have a hysterectomy if I'd exhausted every other option.

I would not choose a hysterectomy as it's quite drastic.  The bleeding might be caused by the HRT if you"ve had scans and they found nothing.

I'm 59, 10yrs post meno, started HRT early 2022 and then had light bleeding over Christmas. GP did exam and referred me, had vaginal scan which showed slightly thickened lining, and then hysteroscopy last week. I'm awaiting results of biopsy now. I was advised that poss result of not enough progesterone and suggested I should have Mirena coil fitted.

I though long and hard, as an unsuccessful contraceptive coil user in the 80s I was a bit hesitant but was assured they are now different, and decided to go with it. I had it fitted during hysteroscopy last Thurs and so far so good! I hardly felt anything on insertion, poss due to local anaesthetic for the hysteroscopy, and no cramps etc after. So I'd def recommend trying the coil.

As for hysterectomy, I'll go for it if biopsy results show any issues. In your situation I think I'd leave myself on the waiting list but see if the coil stops the bleeding in the meantime. If you suddenly come to top of list you can always say no!
