HRT and now Hyperplasia

Morning all,

I’ll try and cut a long story short.

Bowel cancer + pelvic radiotherapy and chemo, = early menopause (38 at time)

Been seeing menopause specialist in Manchester, and I’m lovely and settled on Estradot and Utrogestan, and about to start on testosterone for libido and brain fog.

BUT

Just had a Histroscopy and they found polyps and Complex Hyperplasia, the consultant has recommended a coil and to stop HRT….

Except o don’t want to do either, I’ll revert back to being a monster without HRT and my family were on the edge as it was!!  It’s taken me this long to get onto bio identical HRT, and def don’t fancy a coil for constant progestins

Has anyone had similar and what did you do?! :(

Stick with the regime you are settled on. Don’t change. Polyps can be removed. Do you bleed on utrogestan? If not, stay on it.

The coil is excellent at stopping or controlling bleeding, but if you are happy on your current regime, carry on.

Why do you need to stop HRT?

I understand your not wanting to stop HRT, but complex hyperplasia is the type that can lead to endometrial cancer if not dealt with. it is normally classified as with or without atypia - atypia meaning a type of cellular change which would be more of a worry. Without atypia is less of a worry but still needs attention.

So my advice would be for the time being DO NOT stay with the regime you are on, because this is what is leading to the hyperplasia.

You haven't said what your lining thickness was - do you know this? One possibility is to take a course of progestogens to thin the lining and this would need to be synthetic ones eg norethisterone. I can't see why you would need to stop oestrogen - you haven't said how old you are now nor what dose of oestrogen and utrogestan you are on, nor how long you have been on this regime, nor the symptoms that led to this investigation (I presume abnormal bleeding?) but you do need the polyps seen to.

So - yes have the polyps removed - I presume this has been recommended, ask for a course of a progestogen as an alternative to the coil - such as norethisterone eg for 3 months if possible, but then if you go back to the utrogestan only the same thing may happen. Once the hyperplasia has been  treated maybe you could go onto a higher dose of utrogestan - if you can tolerate it? If not then a synthetic one would be the way to go.

Hiya, I’m 42 now, been post menopause 2 years ish. I’m on 200mg Utro for 12 days a month, and 100mg Estradot patches.  Womb lining was 11mm at scan.

Since posting I’ve done some research and after reading some papers by the Royal college of obstetricians and Gyne, I can opt for an elective total hysterectomy! I’m 100% doing this, and going to write my own report as to why I think this is the best course of action for my consultant over Xmas.

I’ve had 3 solid years of being poked, prodded and bits cut out of me, so I reckon total hysterectomy is the best way forward. Much more cost effective in the long run, and less nhs time overall I think! :)

I mentioned in passing that I bled randomly before/after my scheduled Progesterone bleed. Doctor was like “WHAT??!” Then she grilled me within an inch of my life about how/what/when/where! I wasn’t even going to mention it, as it (seemed) very trivial!

I had no clue that it was big no-no when post menopause!

Hi if you’re post menopause you can take the Utrogestan daily which should result in no bleed. Has anyone suggested this ? x

Sorry to keep asking questions but as with most things female medical there’s often no common knowledge; does a bleed mean red blood or would it also be old brown blood?

 I seem to sometimes be getting random brown at the moment but assumed it’s old blood occasionally being let out with mucus etc.

I have had hyperplasia and five hysteroscopies. My consultants have been fine with me staying on HRT, as long as I have a Mirena coil, which I haven't had problems with. Utrogestan never seemed to work for me and I wasn't ever on a high dose of Oestrogel: the most I ever had was three pumps, for a period of around five months. The rest of the time (just over three years now) I have been on two or 2.5 pumps a day.

The situation may change following my next hysteroscopy, as this has been going on for some years and the team would like to discharge me and may feel the only way they can do so is via a hysterectomy. But the Mirena certainly worked far, far better on my endometrial lining. Could you ask if you could stay on HRT with a Mirena?

Nobody has ever suggested coming off HRT. I've made it clear that I need to stay on in order to be able to work and function.

@ Clarella I also experienced this a few months back dark brown old blood looked like it had come from when I had my last bleed it was a few spots and lasted about 3 days and not again, seemed like it was sitting up there somewhere and was let down with a wee  :-\
