Utrogestan dose help

Hi Ladies, sorry in advance for the long post. I’m new to all this and hoping you can help. I’m 47 and peri, I was on evorel sequi patches for 9 weeks but the side effects got progressively worse, the bloating was on another level to the point I couldn’t really eat much. Gp has now given me estradot 25mg patches (started 3 days ago) and Utrogestan vaginal capsules 200mg to be take for 14 days. This is where I’m hoping you lovely ladies can help. From what I’ve read on here most of you that are taking Utrogestan vaginally seem to be only taking 100mg for 12 days. I couldn’t speak to my gp but have spoken to a couple of pharmacists, the first didn’t seem to know much about hrt and the 2nd said that it’s 100mg only when being taken everyday not on a cycle  (which I think is incorrect or am I wrong?) I’m actually a bit scared to take the 200mg for 14 days and not sure whether too. Any advice would be much appreciated 😊

I'm not very experienced with this but my prescription was 200mg Utrogestan vaginally for 12 days which I only started last month.  I had a very heavy bleed which may not be related to the HRT at all, who knows.  Anyway when I went to collect this months prescription for the vaginal utrogestan, the pharmacist refused to dispense without some clarification with the GP or consultant as Utrogestan vaginally is 'off-licence' for HRT.

I've ended up with 100mg oral Utrogestan capsules which I understand can be used vaginally anyway but will need 2 of these each night for 12 nights for second half of the month. I think 200mg is advised to make sure full protection from the womb lining building up if not having regular scans.  I have read some are only using 100mg vaginally but they maybe privately being monitored.

Hi thanks for replying. It’s good to hear that someone else is taking that dose vaginally,  I’ve been getting myself in a panic about having to higher a dose (if that’s even a thing 🤷‍♀️). I might give it a go and see how I get on, although may only do the 12 days instead of the 14 as that seems to be the norm.

Hi the licensed dose is 100mg every day for post menopause or 200mg for 12 days in peri menopause. And the licensed dose is taking it orally.

Off licence , the Utrogestan can be taken vaginally at the same doses, although some specialists say that you can halve the dose if taken vaginally. x

If you Google 'How to take your Utrogestan dose' with Dr Sarah Ball' on YouTube you'll find that only 100mg is required when taken vaginally. It is going directly to the area so can be half the dose. I am also on a 25mg patch and take 100mg vaginally on days 12-25. This is off license in the UK but has been deemed safe. 25mg is a low dose anyway

That is what I believed going off what I have read on here - that you only need 1/2 dose when taken vaginally. My gp explained that it is classed as off license but that lots of women have had success taking it this way to reduce side effects. I’m just a little confused/concerned about the amount

Your GP is correct. I have had no side effects. so many women take it this way and have scans that show the lining of the womb remains thin. Ultimately you have to feel comfortable with your choice.

On a low/medium dose of oestrogen the half dose is 'probably' OK. On a high dose imo it's safer to stick to the licensed dose which is 200mg whichever way you take it. Those who advocate the half dose have private patients who are most likely having scans anyway but as an NHS patient you won't. I may be wrong but I assume the manufacturer has done more testing than anyone else and perhaps they have found cases where the half dose is inadequate.
