HRT issues / questions

I was recently diagnosed as menopausal after having mood swings, bad anxiety, paranoia etc. I was on a low dose of anti depressants for anxiety which were working OK until very recently.  I had also lost all my confidence, even though prior to this I was very independent (eg I have travelled to many other countries on my own). I was still taking contraceptives which masked other potential menopause symptoms. After a mental health assessment, I was advised to have the FSH blood test as they did not believe I had a mental illness and that my symptoms were due to the menopause. For that reason, I don't know if my periods would have stopped without taking contraceptives. I came off the pill to have my blood test which showed my results as over 30mg and therefore already in menopause. I'm 46.

I then started on Novofem tablets around 3-4 weeks ago and I started on the red tablets (estradiol) and I initially started to feel better, around 80% better. I say only 80% as I definitely wasn't back to my old self yet. I changed to the white tablets around a week ago. These contain both estradiol and norethisterone (progestogen) and I've gone downhill again. I wanted to go out for a solo hike today (something I did regularly) and instead, I stayed home because I feel unconfident, insecure about leaving the house and now I don't really want to do anything. I had other things scheduled for this week that involve interacting with other people and I've cancelled them, as when I feel like this, I can't get myself in the mood for anything.

I also take 50mg of setraline anti-depressants and the GP said he could increase them but I don't want to do that as I've had higher doses of that in the past and it didn't make much difference and gave me more side effects which I'd rather avoid.

Has anyone else had an issue with the progestogen tablets and not the estradiol ones?

Hello Tulip. It is common for women to feel low and depressed on progesterone. What I have noticed is, it is worth trying different types to see if one can be tolerated.

I am sorry to read how low you are feeling. It is really miserable especially when not knowing why.

Thanks - it's unfortunate that I was suffering with those symptoms before all this and hoping I would get rid of them. The GP said I won't have a review for 3 months :(. There is a specialist HRT nurse at my practice so I'll be going to see her next time.

I know HRT is reviewed after 3 months but if you are feeling bad you can ask to be seen sooner. It is unfortunate you were suffering from these symptoms before. Depression has a lot to answer for. :(

It isn't uncommon to have problems with progesterone and if this one doesn't suit you perhaps try something with a different type. Have you needed ads for a long time or is it possible the anxiety is a meno symptom? My anxiety and insomnia were from ostrogen deficiency and took 3 months to go so you should see more improvement yet. Of course some people need both long term but if you feel OK after 3 months on HRT you could try reducing the ads. If you really feel bad on the combined pills I wouldn't wait 3 months to see the hrt nurse.

I have been on setraline for a few years due to anxiety and it was managed well. However, in the last year or so, the anxiety got much worse where I was worrying about irrational things and it was having an effect on my relationship. I ended up getting a mental health assessment because my symptoms were so bad. I'm not back to being as bad as that but I was starting to feel normal again after 2 weeks on the estrogen tablets and then I had to start taking the progesterone ones and I went downhill again :(.

I have ordered some ginseng supplements to see if they help as it'll be a wait to get an appointment with the HRT Nurse.

I can understand this as I feel I can cope until I take any progesterone. It sounds like it may have triggered your anxiety. I hope the ginseng helps.
