Bleeding on Continuous HRT

Hi All, I started on HRT in March, 1mg Sandrena gel & 100mg Utrogestan daily, so continuous regime. I'm 55 and my last period was 9 months before starting HRT. My symptoms before HRT were hot flushes, night sweats, awful anxiety, mood swings etc. The HRT started to help almost straight away and everything was going swimmingly until I had a small bleed in July. Since then I have had period like bleeds on average every 25 days, along with the bleeding I also get all the PMT like symptoms I had when I was still having periods, cramping, sore boobs etc.

I can't get to see my menopause specialist until the end of October and I don't know where I go from here or what has suddenly decided to go wrong. Is it possible I need a higher dose of Utrogestan to control the bleeding? I had problems with breakthrough bleeding years ago when I was on the pill. The only thing that stopped it was taking Norethisterone for a few months. My GP would not let me take it for longer than that. Once I stopped, the breakthrough bleeding started again, eventually I gave up and stopped taking the pill. I really don't want to have to stop taking the HRT as apart from the bleeding its still working really well, but I don't want to go back to having periods again. I've had enough of that!

Any advice would be really appreciated

Dear Beadygirl,

I have no experience of continuous HRT but as far as I know you should have no breakthrough bleeding. I would agree with you, however, that given your symptoms pre-HRT, you should ensure that perhaps it's just a matter of dosage of the HRT. Let's see what others with more knowledge here have to say; another option is to do a search on this forum on this topic - I've done this before and found it useful.

I am the same, for the last 4 months I have had a bleed every 21 days, however this month feels like a real period, the pain is so bad I had to leave work early but so far the bleed itself has not started. I have no answers as I am as much on the dark abut my own symptoms, but will be following this post with interest. Good luck hun xx

Hi Beadygirl &  :welcomemm:

I believe it can take up to 6 months for bleeding to settle down when starting a new HRT regime.
Keep notes of what bleeding you are having & then you can show it at your appt in October.

I don't think it is anything to worry about but if you are concerned or it changes at all then get in touch with your GP.

Maybe it’s too soon to be on a Conti regime? You are not meant to start that until at least 12 months after your last period. A sequential regime may suit you better? Failing that, there is the Mirena coil to consider for bleeding issues?

I agree with Nas but I would also ask for a scan. Most likely it's hormonal but best to make sure nothing else is going on. Some women aren't meno til their early 60s.

Hi i am the same am on oestrogel and 100.g ultrogeston im 8 months in on this hrt and my bleeding as not settled acually getting worse ive now been spotting and some heavy bleeds constantly since 20th aug i have been referred back to see gynecology which my app is on the 19th sep i was seen by them 8 months ago where i had a polyp removed, ive decided today 2 stop my hrt as my bleeding is so heavy today and terrible pain i do think its the hrt im on as been on hrt for years and im only like this since changing to this one

Me too...waiting for a doctor call this morning...you really need to get checked though...scans and samples etc just to make sure all is ok/womb lining not too thick.  I've had all those however and they still don't know why it's happening and talking of changing HRT which I don't want as settled on this one

Thanks for asking Cazikins. After quite a bit of toing and froing and taking into account my history of breakthrough bleeding on the contraceptive pill, the plan is to take a course of Provera 10mg continuously for 3 months to, in her words, blast the lining! Which hopefully will do the trick.

If I'm still bleeding after that I will have a scan and if no cause is found consider a Mirena Coil,   :-\  which I really don't want. I was offered it years ago when all the scans, checks and investigations of breakthrough bleeding on the pill found no cause.

So we will give it 3 months and see what happens

Hi also seen my gynecology last week concerning the bleeding i been having on ultrogeston for the past 8 months ive now been taken off them she said its a side effect from them ao i have changed my hrt

I have been taking the Provera 10mg for a month and have been bleeding for 20 days, some spotting and some days heavier. I have contacted my menopause nurse to check if that is normal. She has now suggested instead of taking one Provera per day for 90 days to take 3 per day for a month. I don't really understand why or what the Provera is supposed to do. It doesn't appear to be doing anything, in fact its making things worse.

I though it was to stop the lining building up, I was previously taken Utrogestan 100mg daily but that wasn't controlling the bleeding. I'm so fed up with all of this now.

Can anyone explain it to me please?
