Hi All, I have two wombs
« on: August 13, 2022, 04:52:26 PM »

Right now that I have your attention folks ;)  I am 4 1/2 months in to my HRT 'journey' and unsure whether to keep on with it or not.

I am 51, and 29 months post menopausal. I was diagnosed with fibromyalgia earlier this year -I thought HRT might reduce the muscle pain. I think it has helped somewhat as I now don't have to have a bath 1st thing to relax my muscles.  I had brain fog/memory issues and serious fatigue for apx 12 years prior to my fibro diagnosis.  Now looking back I wonder if this may have been peri kicking in (when it started I assumed it was stress-related due to the job I was in at the time).

I started on Evorel Conti 50mg and stuck it for 3 months despite increasing anger and tearfulness over months 2 and 3 as I hoped the mood issues might stabilise.  Back to GP, said think I might be progesterone intolerant cos of the constant desire to break crockery, I'd like to try Utrogestan as that seems to be best tolerated generally. So now have been on Estradot 25 and Utrogestan 100mg 25/28  for 6 weeks. I also take fluoxetene 20mg daily, for 12 years plus. Gp has suggested memory issues may be due to fluoxetene but getting to grips with fibro was hard so I didn't want to stop an antidepressant then.

The Utrogestan is causing digestive issues/ibs type symptoms, but my mood is also badly affected in that I regularly burst into tears.  My mood was much more stable before I started HRT (pesky hormones!).

I see Utrogestan can be taken vaginally to cut out/reduce digestive issues (and perhaps also reduce my paper hanky consumption  ::)) but with 2 wombs in the picture I assume I would need to apply to both, and alternate. Intuitively it feels as though taking 4x as much progesterone as oestrogen is unbalanced, but maybe that's just my perception.  Also I wonder if my progesterone dosage would differ as I have 2 wombs.

I am thinking of going back to gp, to see meno nurse this time, and seeing what they suggest. I would really appreciate if  anyone has been in a similar enough situation to mine to shed any light on next steps for me?

Phew that was a bit of an epic, thanks for reading if you got this far!

Bye for now,
Chucklesista  ps not sure how to get the wee 'new' label by my post.....

Hello chucklesista and we to the forum.

When reading through your post I realised that you really do have two wombs! I have heard of this but I don't think that it has come up on the forum before.

Unfortunately I can't help with your particular problem but I do sympathise as mood issues continue to bother me even though I am on a different HRT.  It has also been suggested to me that sometimes too much Oestrogen can cause as many symptoms as too little so your comment about being more stable before HRT also resonated with me.

I wonder if you should be referred to a meno clinic, given your anatomy? You could also email Dr Currie who established MM and the details are on the site.

I am sorry that I can't be of more help but we have many knowledgeable ladies here and I am sure that they will be along to advise you.

Wishing you well and take care.

What is your anatomy lower down? 1 or 2 cervixs/vaginas? According to Louise Newson but not the manufacturer you can halve the dose if used vaginally so 100mgeevery other day. If they are separate perhaps use 100mg each day in alternate wombs. The other option might be a long cycle (6 week) sequi regime to reduce the time you spend on utro. This is what I do as it gives me fatigue.

I think you might need very specialised and expert advice with this one!

You may want to consult with a few different specialists and hear their recommendations and then decide which to go with.

You could have two Mirena coils, for example....  ???

Interesting ........

Who has over-seen your gynaecological history thus far?  I am sure that there will be a support group somewhere 'out there'.  I would compose a short e-mail to Dr Currie to ask the question.

Browse round.   :welcomemm:

Hi and thanks for welcoming me to the sisterhood of hormonal upheaval  :)

I shall get in touch with Dr Currie as you suggest. And I suppose there is a support group for all ills nowadays isn't there CLKD.
I really hoped that there would be one other person on MM who had been in this position, but I guess by now I should be used to being the novelty act. Very popular with students sitting in, 'oh this lady has something rather unusual'  ???

Your suggestion re using Utrogestan was what I had thought of, Sheila 99.
I do have two cervixes, one is less accessible than the other, so I've had my smears done in hospital.  As I've never had kids or any other major gynae issues, apart from some dodgy cells on my cervix that needed whipping out via LLETZ, I have only seen the consultant who's done my smears.

The thought of 2 mirena seems sensible joziel, but makes me feel queasy. I don't want any medication I can't control if I'm honest, and the probs they have doing my smears in the 'small' side mean it would be a right performance inserting a coil into that womb too.

Would I be able to do a long cycle sequi post-menopause via the NHS, do you think, Sheila99? I'll see if I can be referred to a specialist by my gp practice too and also have a good browse round MM.  There's so much useful info but I am loath to start new threads when others have already been in a similar(ish!) position.

Onwards!

Let us know how you get on?  Would it be necessary to have two coils?  How 'active' is the smaller womb?  A scan to see if there is any build up might be enough.

How were your periods?

My sister has 3 kidneys  ::) - 1 is normal size, the other has a 'branch' which is almost a fully sized kidney.

It depends if you get a 'by the book' or a 'what's best for the patient' type but they aren't watching. Hurdity does it with medical supervision but her gp is (was?) a gynae. Mine would freak out but I've always had a 6 week cycle and fitting it into a 4 week hrt cycle in peri just didn't work for me - so I reverted to 6 weeks and am staying on it. The important thing is that your lining remains thin. Mine doesn't seem to build up much and I only have a proper bleed in about 1 cycle in 3. You may build up more so it may be wise to scan after a few months to check it's OK, you can do it yourself for around £100 if you can't get it on the nhs. Time it for just after a bleed.
You may also consider provera instead of utro, it's oral but may suit you better than oral utro.
It might be worth asking your smear consultant if he can suggest anyone with experience, I imagine there will be specialists who have no experience of this.

It's probably best to use All Things Menopause I reckon. I will only use that one from now on. Thanks for suggesting this, I have never used a forum before and it is frying my brain a wee bit tbh.
