No bleed on sequential HRT

Hi,

My GP reviewed my HRT today. I’m now starting cycle 7 of 2 pumps Oestrogel and 200mg Utrogestan day’s 15-26.

We are both confused because I have only had one light bleed (cycle 2).

I had missed 8 periods before starting HRT.

GP going to review again in three months and if no periods GP will write to specialist to ask if this is normal or if anything needs changing.

Anybody any thoughts?

Should I ask a menopause specialist?

Hi it could be that you are actually post menopause and would be better taking the Utrogestan every night. x

I have the same problem, 4 pumps gel, 12x200mg utro, long cycle, late peri. I have few cycles of my own now (3 in the last year) and when my own body doesn't contribute I rarely bleed. It seems some people don't always bleed but I find it a bit worrying. As an nhs patient I don't have scans so wouldn't know if my lining is building up. My gp is clueless too and I'll have died of old age before I get an appointment with a specialist. A mirena would solve the problem but I'm not keen on something I can't remove myself if I feel I need to.

If you find the answer please let me know.

I’m the same too, 4 pumps gel, 12x200mg. When I started HRT in late July/Aug I’d missed about 4 periods and don’t always get them after finishing Utrogestan for 12 days, most of the time I don’t.
I’m too far away from my surgery to get help so am considering a private scan. Not keen on getting a Mirena either.
I didn’t get a period on 2 pumps or 3.

Dotty - thanks for replying. I have wondered if I’m now menopausal and whether I’d be better on continuous regime, would I need a good bleed beforehand? To ensure womb lining is thin?

Sheila - thanks for replying. I’m sorry you’re in the same situation, I too find it a bit worrying as I haven’t had scans either. I’m on NHS too. I feel happy to wait another three months to see and am glad that my GP will write to a specialist to find out, but I wonder how long that would take.  I ponder the mirena too but I’m the same as you, I wouldn’t like not to be able to take it out if it wasn’t suiting me.  I also wondered about long cycle and see that you are still having this problem on that. Is this an indication that we’re now menopausal?  Or do menopausal ladies get a bleed if on sequential regime?

If I get an answer I will let you know.

VictoryV - thanks for your reply.  It’s good to know I’m not alone experiencing this issue but I’m also sorry that you are having this too. I’m not keen on mirena either. I’m not doing too bad with this regime so far but I would like to know why I’m no bleeding and if it’s normal or safe to not bleed. It may well be that I just haven’t developed thicker lining of womb? I don’t think it helps when GP doesn’t know either.

It’s good that you’ve got a GP doing a review. I think Hurdity has written that lack of bleeding on a sequi routine is better than abnormal bleeding but I can’t find the posts to link!

Maybe we’re all nearer post meno than we initially thought! My Mother pointed out this morning that I’m menopausal, peri menopause is b*ll8cks, job done, now barren, get on with it as it’s natural! Thank God the English Channel is in between us!

I think you’re right about the lining not being thick enough yet. I’m not worried so I’ve not booked a scan. I’m going to see how the next 3 months go too.

Hi VictoryV - that’s reassuring that you remember Hurdity writing about that.

Yeah, I actually thought I was getting to menopause as I’ve been peri for many years and now things had changed moreso, especially missed 8 periods on the run.  My mum had a hysterectomy so I haven’t had any advice from her., though she is sympathetic thankfully.

Yeah, my GP had a good chat with me about it and we both decided that it was worth waiting three months to check if I bleed, then at least I know he’s going to ask for help from a specialist.

I think most women do continue to bleed on a sequi regime when they're meno. Being bleed free seems to be the reason most choose to move onto a conti regime when they're meno. It seems a bit of leap of faith to me to assume you don't bleed because the lining's thin (particularly for me on 4 pumps) as I would expect it to build up. Perhaps your gp will send you for a scan if you still haven't bled, it would be nice to know what's happening.

I agree with it being a leap of faith Sheila, I’m being overly optimistic!
I’m going to book a scan only because I feel I can’t speak to my GP and I don’t want to reduce gel. I’m not worried, I’d just rather know.
Does getting a scan mean we then need a specialist to interpret it or is a measurement given with the scan? I do hope it’s the latter!
