Belle1, sorry to hear about your migraine auras, they are scary and debilitating but not dangerous.

I started getting silent migraines (aura but no headache) at the onset of the menopause - I had a Mirena coil at the time.   It took me years to find out what my personal trigger is but with the help of a hormonal migraine specialist, it would seem they are caused by oestrogen spikes and hormone instability.   Obviously you experience a lot of hormone spikes in peri menopause but the spikes can be made worse by progesterone and having a cycle. 

Migraine with aura is usually caused by oestrogen spikes but you said your auras happen around patch change time so it could be generic hormone instability which is a major cause of all types of migraines.   

I was advised to keep hormone levels as stable as possible and to avoid a cycle.   You mentioned Femseven 50 patches, are they oestrogen only?   The Femseven conti patch that came loose would have undoubtedly caused fluctuating hormones which would have been a migraine trigger.   

I'm afraid I found Utrogestan caused my oestrogen to spike because the dose was too high for me.   You might be better with a hormonal coil.

Belle1,I use bespoke progesterone at a 50mg dose continuously with Oestrogel and it works very well.   Unfortunately, if you have bleeding problems, this dose will not work for you because it is way too low.

It's a pity the Mirena coil didn't work out.   I wonder if you should try a different patch or gel to get a more stable dose of oestrogen?   

I doubt the migraine aura has anything to do with iron deficiency.   That said, it might be worth having a blood test to check your levels and at the same time get your oestrogen levels checked because they might be too low and it could be causing the auras.   

In the meantime, why not try changing the patch a couple of days earlier, it might help to keep your oestrogen levels stable.
