Help needed!

Hi
I am 41 years old and have been suffering with horrible symptoms for about a year now. Terrible anxiety, extreme fatigue, muscle pains and aches, mood swings (Jekyll and Hyde), crying over silly things. I think I’m perimenopausal but my doctor doesn’t as my fsh test was normal once although my oestrogen was slightly low. My mum was similar age to me now when she went through it. I just wondered what I can do when my doctor says it’s not menopause related. I need some help because my relationship is suffering and my health is abhorrent, some days I don’t move off the couch.

Sorry to hear about your symptoms and also the effect these are having on your relationship.

Re being peri-menopausal - the first thing is what is happening to your cycle as irregular cycles and especially having slightly longer cycles  - are the first indication that your are becoming peri-menopausal. If at your age you are having irregular cycles, then yes blood tests should be carried out, but as you are only 41 the NICE Guidelines say that two tests should be taken 4-6 weeks apart to look at FSH levels in particular. Having said that this does vary so a low reading does not necessarily indicate that you are not peri-menopausal. However I think raised levels combined with irregualt periods are needed to diagnose early menopause. If this is the case then you would be advised to take hRT to help protect heart nad bones in the long term.

Another possibilty is to take the contraceptive pill which will give you extra oestrogen and at the same time regulate your cycle to eliminate the mood swings. There are a couple which are similat o HRT which docs can prescribe - one is called QLAIRA and the other ZOELY. I think they are still avaliable. With Qlairia the oestrogen dose varies a bit throughout the month, and there are only 2 days with dummy pills so no week long break.

There is a section also called Help for Husbands on the forum somewhere and also on the main website "The Man Shed" - so maybe something to tactfully show your partner?

Thank you so much for your advice. My cycles are shorter than they used to be and also they are really heavy for 2 days and then not, just very different to what they used to be.
We cannot currently get an appointment to see our G.P’s and it’s so difficult to have this type of conversation over the phone so I’m going to have to put up with this for a bit longer until I can arrange a private appointment or finally see a G.P in our area. I feel absolutely horrific on and off and I just cannot put up with it much longer. I have been tested for everything else such as thyroid etc and they’ve all come back clear. I will research further online and see what I can do next. Thank you.

Hello KayB and welcome to the forum.

One of the first questions my GP asked me when we were discussing the menopause was 'when did your mother go through it?' as this was a good indication of when my menopause would begin. 

Can I suggest you approach your GP again and remind him of your mother's experience. Also many doctors only think of hot flushes and night sweats as symptoms so even if you have not had these perhaps say that you have so that he takes your situation seriously.

Your GP is talking rubbish - another one who hasn't got the message?

You know your body and you are peri-menopausal.  Maybe keep a mood/food/symptom diary to discuss with your Practice Nurse who may have more knowledge than your GP.  Then see another in the Practice.  Your blood tests are within normal limits and quite frankly, blood tests are reliably unrealable!

You don't have to put up with it.  Your Surgery should be seeing patients by now even if it's whilst you are in your car and they talk through the window!  Make that suggestion or better still, ring and ask for an appt. to a menopause clinic.  You shouldn't have to pay but sometimes, with the lack of knowledge in many surgeries, it is quicker to get proper treatment  :bang:

Eat little and often.  Rest.  Keep hydrated. Print off 'hints for husbands' and let him read it ;-).

Just to say that your doctor is probably right that you are not yet peri-menopausal according to the medical definition. As your cycles are getting shorter you are in what is known as the "Late Reproductive Stage" ( or maybe it's phase?) which is the last stage before peri-menopause hits. This is apparently classic. However at this point hormones do begin to start to go awry even if FSH is mostly normal, but ovulation will probably be occurring regularly. As the ovaries start to fail then cycles begin to become sporadic as you enter the peri-menipausal transition. This ties in with the age when your mother went through it and indicates that peri is probably about to start sometime soon....

From what I've read though, this phase can be very challenging (the stage you are in) and pms can become worse. However because oestrogen will still be circulating sometimes at high levels, I suggest HRT as such would not be indicated although I know some women do start taking a low dose at this point.

You really should not need to see anyone privately but in your position, if you are really struggling, I would seriously consider the pill options I mentioned because they regulate the cycle and will stabilise your hormones after a few cycles. This will tide you over during the forthcoming turbulent years and take you through the time hopefully until menopause hits and if you want to you can then transfer to HRT?

Surely you can insist on a face to face if you need it? Alternatively, if this option appeals to you, is there somewhere you can ask for contraception because in essence this is what it is - the pill - where you can get advice and a prescription. Be sure to research the different types and if you decide on something like Qlaira make sure you  insist on trying it.

Other options which can help with the bleeding are the Mirena coil, and then if you find you need extra oestrogen - this can be added at a dose which alleviates your symptoms, and increased as needed as the years progress?

Thank you to everybody for your help. My hot flushes can last hours, not minutes, it’s horrendous. I’ve read online that you cannot take contraceptive pills if you’re over the age of 35 and smoke. I don’t smoke any more but I still vape, that is my one naughty thing. I will try and arrange for an appointment with a different G.P but I don’t hold out much hope as a lot of G.P’s are the same. There is also a very long wait to see a menopause specialist and if my doctor doesn’t think it’s menopause related, I doubt he’ll refer me. I think it might be worth paying for a private referral to at least speak to a specialist in this area. Thank you for your advice everyone, it’s nice to hear from people who understand.
