Not getting a withdrawal bleed on Utrogestan this cycle..?
« on: June 29, 2020, 10:00:04 AM »

Hi ladies, looking for a little advice plse on why I'm not getting a withdrawal bleed /my own period 5 days after finishing the Utrogestan?
Started hrt 2 mths ago, using v low dose (half  pump) Oestrogel ( in fact Oestrodose as I discovered.. ) and 100mg Utro orally for days 13-25. Was still having regular ish periods before HRT, anywhere from 20 - 30 days each mth.
On first hrt cycle I bled only a few days into the Utro, v early on day 19. This cycle however I have taken the whole 12 days and now 6 days after finishing it still no sign of my period!  >:( (And as a result have dreadful insomnia and not feeling too chirpy! )
So do I just wait for it to come no matter how long it takes and then start counting first day of bleed as day 1 or should I already have been counting a new cycle from say day 28 of this cycle?  I'm just concerned I could potentially have a v long cycle going on here and therefore will be taking relatively little progesterone in relation to the ( albeit low dose of) oestrogen?? Does this matter?
Any advice much appreciated- trying to get gp advice such a long process at mo.

Its probably because you are on such a low does of gel, that you  have not built up enough lining to shed. I would continue as normal with your gel and if you dont get the bleed start the utrogestan at the next date you are due. I go for sometimes up to 10 weeks with only the gel as I also use a very small amount just under 1 pump and even on that when I get a bleed its very light compared to what I used to get, the older we get the less we seem to shed. :-\ Also to add, perhaps try using it vaginally, it tends to be more effective than orally as it doesnt bypass the liver etc.

According to the manufacturer you should use 200mg for 12 days. If you use less it should be under the care of a specialist with regular scans to ensure your lining isn't building up. Not sure if this has already been discussed? I am in the same boat atm. I've bled twice from the last 4 cycles and those were ones where my body contributed. I am on 3 pumps so more chance of my lining building up and as I'm now late peri there will more cycles where it's only utrogeston bringing on a bleed. I would do as Cassie suggests for now. I'm not sure what the long term answer but I'm certain my gp won't know! My anxiety and insomnia took 3 months to go. Have your other symptoms gone? It might be that you need more oestrogen.

Thanks so much ladies, that's really helpful.
As you pointed out Cassie my lining may not have built up much esp as my last period was unusually heavy during my first HRT cycle. It's just odd and annoying that my own cycle isn't bringing my period on. ...??
So if I understood correctly, for now I should calculate when mid cycle would be had I had a period on say day 28, and begin the utro again then?
Sheila99 I'm later peri too. (I recently did a pico Amh test which said I have a 75% chance of final period within 3 years tho I'm expecting it to be sooner.)  I originally saw a specialist 2 yrs ago and was prescribed evorel 25 with utro 100mg just for 10 days!! I decided to quit HRT bsck then however ( felt awful on patch) and have now restarted under gp with gel. Gp not v clue up on dosage but we're basing it on original specialist recommendation hence the 100mg as I'm on such a low dose.
Thnks again ladies, nice not to feel so alone with decision making in all of this as finding it stressful. The insomnia really doesn't help, that's the main reason I decided to retry HRT as been debilitating at times. I found the utro actually helped a bit this cycle and almost felt human some days. I must be one of the few who actually prefers the progesterone phase!!
