Newbie with 2 main problems
« on: March 18, 2019, 02:44:57 PM »

Hi all.
. Just joined. At the grand old age of 56 I am still annoyingly Peri Menopausal! I had 3 periods last year and I've just had one ! Annoying. Anyway apart from the sweats I have 2 problems really bothering me so any advice would be gratefully received. Bearing in mind I've had breast cancer so cant take any Hormone treatments or supplements which mimic Oetrogen.
Prob1- A wound up, taught feeling, like a guitar string. I feel so short tempered and angry and I'm snapping and shouting at my poor hubby all the time. Its like having permanent PMT I was so wound up and unable to relax on Saturday night that I actually fell asleep at 6am Sunday Morning after 2 hrs of solid meditation!! I've had anxiety probs and Panic Disorder for years and whilst the anxiety has increased this isnt it,. I just want to be pleasant for my poor long suffering hubby who is also my carer and doesnt deserve me biting his head off all the time!
Problem 2- A sore Vulva. I know drying up Oestrogen dryes everything up but I actually feel sore aswell as dry.  I bought one of those ridiculously expensive for what it is Litmus lollipops to test for BV and Thrush and I treated myself for Thrush just incase but I know its menopausal. As I say I dont think I can have Oestrogen cream but I'm using a vaginal moisture replacement called Sylk.
Any help would be great!

Hi and welcome cinders 64

Problem 1:  It is absolutely normal to feel “wound up and and short tempered” - all part of the delights that is the menopause.  Great that you do Meditation but it can only help so much.   I can really relate to how you feel and I spend so much time trying to control this horrible feeling - I go through days when I just want to scream at everyone and because I don't scream at everyone I'm a tight spring of tension.  I do a lot of walking which is my best help - exercise is probably the best way to relieve things.   Some find Valerian helpful, perhaps St John's Wort? I'm sure the GP would dish out anti depressants to you but I personally don't think they are the best option unless you have true depression.

Problem  2 - as Birdy has said, what you need is local oestrogen which isn't absorbed systemically so shouldn't be risky for you. Your GP might want to check with you cancer specialist but Vagiem or Ovestin could really help.  Great you are using SYLK - I love that stuff - try using it more often. To help with sex, I use Durex Perfect Glide
If you do think you have a bit of BV or thrush, then do try using Multi Gyn Actigel - this is great stuff that helps to restore the natural flora balance which treats and prevents BV and thrush - I use it a couple of times a week as a preventative and if I feel sore or itchy (sometimes with a dodgy smell) I use it 2-3 times a day for 2-4 days. I don't use the nozzle, I just put a good dollop on the end of my finger and push this up inside the vagina and around the outside - it does sting a bit at first but it is then soothing. You can buy Multi Gyn Actigel online.
I tend to avoid thrush and other harsh treatments as they can actually make things worse.

Keep us posted.  DG x

:welcomemm:  can't add much other than consider Quality of Life.  BC shouldn't make HRT a no-no.  Not enough known and clinicians tend to err on the side of caution.

I have used Ovestin for several years, VA for me was like razor blades up there  :o with urine infection-type symptoms - samples came back with no growth and suggested localised HRT.

Let us know how you get on.

Hi Cinders64

 :welcomemm:

Just to add also that the British Menopause Society has recently published a paper - a consensus statement for the management of oestrogen deficiency symptoms in women treated for early breast cancer (which is quite technical and intended for specialists) and have produced a summary consensus statement here:

https://thebms.org.uk/publications/consensus-statements/the-diagnosis-of-the-menopause-and-management-of-oestrogen-de%ef%ac%81ciency-symptoms-and-arthralgia-in-women-treated-for-breast-cancer/

Key points

An early menopause, estrogen deficiency symptoms and arthralgia are common side effects of systemic breast cancer therapies.

Symptoms may persist for the duration of treatment and in some cases continue after treatment completion.

The NICE Menopause Guidance [NG23] recommends referral of women to a healthcare professional with expertise in gynaecological endocrinology for counselling about the risk of developing an early menopause and the management of symptoms associated with breast cancer treatment.

Lifestyle measures and non-hormonal interventions should be first-line management for estrogen deficiency symptoms but if these are ineffective systemic hormone replacement therapy or low-dose topical estrogen may be considered but only after taking specialist advice.

Lifestyle measures such as weight loss and increased exercise may also improve arthralgias associated with aromatase inhibitors and tamoxifen

Switches to endocrine breast cancer treatment may alleviate symptoms of estrogen deficiency and arthralgia but these should only be instigated in secondary care by the breast specialist team.

Hope this helps

I do wonder why, if HRT - which many women will require - it isn't recommended that the breasts be removed?   :-\

Hi Cinders
Welcome to the late menopause club! I'm in it and several others are too! You might find my old thread helpful
https://www.menopausematters.co.uk/forum/index.php/topic,36765.0.html

I found PMT to be absolutely horrific especially in recent years when it would go on for weeks.
