Completely confused

Been told by NHS consultant to increase my estrogen intake to Evorel200 - I thought that was a really really high level, the patches only go up to 100. And at the same time I'm not taking any progesterone at all, to try and determine whether my persistent symptoms of acute anxiety are down to progesterone intolerance.  Hasn't made any difference at all apart from constant light bleeding, in fact I'm finding that I can't cope with estrogen above about 125, I start feeling a horrible kind of agitated depression which is exhausting and miserable.

What's really confusing me is that blood tests have shown that on Evorel 100 my estrogen level remains around 1000 pmol, which is a pretty high level. And yet two different specialists tell me that they think it's highly unlikely that this dose is keeping my level that high (after chemo I'm not producing any hormones of my own). I really don't know what to do now - do I stick with the estrogen increase although it makes me feel awful, do I restart utrogestan or do I just give up on the whole HRT thing and hope that time just sorts everything out? The way things are at the moment I can't actually live a life - anxiety and depression rule the roost.

Hi racjen - your post seems to have got missed - and perhaps because it is a difficult question to answer and most of us feel powerless to help you whilst wishing we could and feeling for you! You are being seen by specialists but there seems to be disagreement as to the best course of action. If the specialists are not believing your blood results from 100 mcg patch then this is tricky. Where do you apply your patch and when in the patch "cycle" have the blood tests been done? In your position and because of your ongoing issues I would ask for a couple more blood tests - both to be taken about half-way through the patch-change cycle eg if you change on a Monday night then perhapas Weds morning - so you can see what the average is for you - and also at the same time of day.

If you feel worse when you increase the oestrogen then surely this can't be the answer. You are right that 1000 pmol/l is very high and also to be at this level all the time - notwithstanding that women in surgical menopause and those with reproductive depression may need consistently high doses all the time and apparently higher than other women, in order to feel well.

How long have you been without progesterone? The only way to answer whether you should start taking it or not is either be guided by any bleeding - although sometimes this can happen at quite a late stage after endometrial thickening, or ask for a U/S scan - an easy low cost option. If your womb lining is still within normal limits for cyclical HRT then presumably you would be safe to leave it for a while longer - if that's what the consultant has said?

I'm sorry I can't be of more help - you are already receiving specialist treatments and advice and probably none of us are able to advise further in the way you would like but I hope any response just helps you work it out a bit more in your head? It really depends on how you feel and the state of play your body is at right now I would say?

Hurdity always gives such good advice and backs it up.

As you know I'm in surmeno and along with chem meno  can be different  . Not worse just different. I have read that it can cause more abrupt And heightened symptoms, (something i have experienced)  and..

most importantly

.. may require a higher level of hormone therapy initially (born out by my experience) 
so it would stand to reason to me that if I was still having regular as a clock periods and very few menopausal symptoms even at 53 that age is not so much the factor as where you were  in your own biological journey before  surgery or chemical intervention.

So I realy empathise with you about blood test levels. Our own hormones aren't coming into play so blood test levels are important for us. It's realy all we can go by when symptoms aren't controlled.

I wish I could offer more but I'm going through the same thing myself and I haven't got the answers ...yet. so can only share in the hope it helps in some small way.

Do you need oestrogen at all? 

How long have you not had progesterone artificially and is there any relief in symptoms?

Yes, without estrogen I feel extremely depressed  :-\
