Is my HRT becoming less effective?

Last March my consultant and I were finally happy with my HRT regime, Indivina 2mg/5mg and the equivalent of 1 sachet Testosterone per week. He signed me off to the care of my gp for 12 months so I'm going back to see him in March this year.  About 3 months ago I noticed that my symptoms are returning. I'm having occasional night sweats, occasional hot flushes in the day, I'm a bit more achy than I'm used to and I've had a few sudden energy drops again. On top of that I've gained nearly a stone in less than a year which I can't shift despite not drinking alcohol, cutting out sugar as far as possible and walking a lot, albeit slower than before. So what's going on? I dread to think what I would be like without the pills but can they have stopped being effective? I'm 61 and have been on HRT for about 4 years
