Hi ladies,
I'm after a bit of advice. I'm 47 peri and on hrt of mirena coil, Everol 100 and testogel. Things have been pretty settled but I have started to get bladder symptoms every now and then, it doesn't feel like a UTI more just feeling of pressure on the urethra, I also get a sore vulva at the same time. It does seem to be following a cycle of happening just before I have a bleed, which seems to be happening every couple of months. I'm wondering if this is the start of VA and whether I'd benefit from something like vagifem. I also seem to be getting alot of recurrent thrush and know that high oestrogen can make you more susceptible to this, so worry that using local oestrogen will mean more thrush.

Certainly what you have described sounds like VA. Many women need local oestrogen as well as systemic.

I had not heard that high oestrogen can lead to thrush, one would have thought the opposite was true. Declining oestrogen in the urogenital area can upset the vaginal flora, because the vaginal environment becomes less acidic and also less glycogen is produced.  A low vaginal pH is needed to promote the growth of desirable lactobacilli which out-compete the undesirable bacteria and the Candida fungus.

I would go for the Vagifem in your situation, before it gets any worse. You can also use some Multigyn Actigel. It will help to rebalance the pH and prevent bacterial vaginosis.

I would definitely give Vagifem a try now, I needed to use it in the peri stage.
Regards to Thrush, I actually got that more frequently before I started HRT, I am on systemic as well as Vagifem, infact I haven't had one episode since I started on HRT and I am on quite a high dose.
