Utrogestan dose? Think too high??
« on: August 27, 2018, 06:58:17 AM »

Hello,
I've just been prescribed estrogel and utrogestan regime having advised my GP (!) after research in this site and friends.
I'm not 100% sure that the dose is right of either but am panicking about the utrogestan (200mg) as I react to progesterone. Gp said one pump estrogel (I'm taking 2 as recommended starting dose on instructions and also because femoston 1/10 was not enough). This is still a low dose (I believe) and from what I have seen on this site, utrogestan can be prescribed as 100mg and this may be the correct ratio for me? Also, my tabs are oral - does this make a difference?
Thank you so much ladies - this site is amazing and the women and so helpful and patient

Hi are you in peri menopause or post menopause? There are different dosages depending on which stage of menopause you are in. X

If you are peri then the dosage is 200mg from days 15 -26 of your cycle.

Here is the link to the TREATMENTS section that explains how to take Utrogestan:
https://www.menopausematters.co.uk/to_progestogens.php

If you are peri meno then you need 200mg for 12 days each month - capsules can used orally or vaginally - to ensure a proper shedding of the womb lining.  2 pumps of Oestrogel per day is roughly a medium dose.

If you have regular scans, then some women are able to use less progesterone but this would need to be monitored and you wouldn't get regular scans on the NHS.

Hope this regime goes well for you.
Good luck. DG x

Thanks Dotty and Dancinggirl, that's reassuring. Re the dose of estrogel, I actually think my GP was right and I should have started lower (1 pump) as I felt a bit jittery today on 2 pumps.
Thanks again x

The gynae I last saw suggested doing one pump per day for two weeks before increasing to 2 pumps per day. DG x

Mary G
You can't recommend to anyone to follow the Studd regime unless they are under the supervision of a specialist and have regular scans.
Even though my gynae allowed me to do a longer cycle and use 200mg for 12 days every 6 weeks I still got erratic bleeding and there are many women who find Utrogestan problematic.  Utrogestan isn't as stable as other progesterones and this is why the standard dose is 200mg for 12 days each month.
Not everyone can afford private treatment so it is unwise to suggest something that could cause harm.

I agree with Dancinggirl and this is what Dr Currie had to say on this very subject:

Just to confirm, while Professor Studd is very experienced and extremely knowledgeable, his regimens are not always in line with current recommendations, particularly around use of progesterone.
As always with providing menopause advice, this should be individualised and flexibility should be applied, as long as there is a clear understanding of national and international recommendations. Some women are intolerant of progestogen and progesterone and so it is appropriate to adjust the regimens, as long as it has been fully explained and discussed.
Best wishes
Heather

It is incorrect to say "there is nothing wrong with this regime". It may be fine for you Mary G but research has shown that reduced dose and duration of progesterone leads to increased risk of endometrial hyperplasia which can lead to cancer. Each woman will have a different reaction to such a regime and it is vital to only do this under medical supervision. If you are extremely progesterone intolerant then you can ask to be referred to a menopause clinic (on NHS) and then you would be able to be scanned regularly - there are members on here in this position.

Lilylight - with a low dose of oestrogen you may well be fine with a lower dose than licensed dose but always start with the recommended dose, see how you feel and go through your GP if you would like to make changes.

Thanks all - just came back to thread to see latest  replies (sorry). I think Mary G post removed, but I understand the gist - to go through GP and not reduce utrogestan without guidance.
I started the utrogestan today 200 mg (day 14 cycle) and did feel lightheaded and exhausted. I take it orally but am wondering if vaginally May be preferable in terms of containing symptoms. I've read its ok to just insert the oral capsules - I'm hoping that I can insert 2 as mine come in 100mg caps.

Hi Lilylight
I changed from Femoston 2/10 to estradot patches 50 with 200mg Utrogestan from days 1-12 of my cycle.
Like you have 100 mg capsules and I take it orally.
My first month was unreal, I got very depressed low mood etc, dizziness.
This month not as bad and I take it before I go to bed. Made a huge difference to me
Hope this helps.
I still haven't got a bleed but will see what happens as change over I presume can cause irregularities
Hope this helps xx

Thanks golden, that's a good tip re taking at night. I'll try that. Also been eating like a horse today - ravenous. Must be progesterone I think.
Glad your symptoms are settling - fingers crossed for you xx
