WTF? Confusing clinic advice

hello ladies,
I thought I had it all sorted, but things are still not right and I'd really appreciate your advice. Quick recap: I got one proper withdrawal bleed after starting vaginal utrogestan (100mg for 12 days) and after that felt like a new woman - thought it was all sorted.  Since then same old problem that I've had for the fours years I've been on HRT - no withdrawal bleed and really bad emotional side effects plus abdominal pain and bloating around the time when I should be getting the withdrawal bleed. I can barely keep it together. :'(
I spoke to the meno clinic nurse yesterday who said the same thing she's being saying for ages 'it's a hormone imbalance'.  Yup, no kidding that's why I asked to be referred to you  ;D
Yet again she wants to test my oestro levels in bloods - to see if there enough circulating oestro - frustrating as I've had the tests before and I've never had a problem with absorption of the oestrogel.  She says if that's ok then may it's just that my body doesn't want to bleed anymore - frankly don't understand that advice - thought the HRT was supposed to bring on a bleed to protect the uterus cos of course you body doesn't want to bleed anymore - that's the menopause!  I know she is going to suggest my next option is the Mirena coil but I don't get on with progestro or with IUDs so what are my options?  Is something as radical as hysterectomy the only option left.  I can't believe I'm even starting to think about that seriously. 
So anyway I thanked her for being so lovely and supportive (which she has been) but I'd like to be referred up to see a specialist doctor please. She's said she would do that, so......any advice for what to ask when I get the appointment.  Am I just in a crazy fog and over reacting to everything?   Just want to find a regime that works for me so I can have my life back.

Hi were you still having periods before starting the hrt?

Have you tried continuous utrogestan?

Hi Dotty,
I was still peri when I started but periods up to 100 days apart.  Told I am early meno but don't know if I'm officially menopausal cos of HRT. Haven't taken utro continuously - can this help with the emotional symptoms?  Slightly worried because when on the standard oral dose (200mg for 12 days) sequentially I got terrible rages and also felt suicidal within only two cycles of the higher dose.  I've tried the lower 100mgs for 7 days dose too and that didn't work either.

Hi Provera is an alternative to Utrogestan x
