Prolapse surgery.

Had hysterectomy in 2007 for uterine prolapse involving bladder.  Had  prolapse surgery for rectocele in 2015 that hadn't been dealt with during hysterectomy. I can feel my bladder sometimes and see it when I look with mirror so I think it's prolapsed again. Just been through a bout of diverticulitis ( bowel infection) and it has left things worse. My question is that I seem to have air in my vagina most of the time, even before bowel infection, like a constant air bubble. Is this normal ? I have backache too now. Can anyone help with this?

Hi and welcome to MM
This forum is basically for menopause related issues and I assume you are either peri or post menopause ? Systemic and/or local oestrogen would certainly be advisable as it would help maintain a healthier environment around the vagina and bladder. I'm surprised you haven't been adviced to have oestrogen following your hyster?!!!
There may well be ladies with your issues who may be able to advice.
 I had diverticulitis last year and it took ages to recover - it's horrid. I had ongoing back pain and I eventually went privately to see a rheumalogist and they discovered my L3 - L4 vetibrae were compressing the disk which was inflamed - I had a cortisone injection which really helped. However, my diverticulitis caused back and hip pain as well - I think the whole area was inflamed and the diverticuli will continue to be sensitive and cause discomfort even when the infection has subsided I believe.
You need to get everything checked, so see your GP or specialist and I would definitely ask for some oestrogen. Virtually no risks with oestrogen only HRT but there are lots of benefits. DG x

Hi and thank you for your reply. I am post menopausal. I'm 62. I am using vagifem pessaries. I was using them before but had to stop while I had the diver cause it was upsetting my bowel. Back on it now but it hasn't kicked in yet. I have vaginal atrophy too. Extremely unpleasant. It is this air bubble thing that is puzzling me, it could be normal though.

Hi Scottish girl. A big welcome from me.

I had a hysterectomy for uterine prolapse plus an anterior repair for bladder prolapse in 2014. I'm 64 and sadly feel that things are on the slide downwards again so I can fully appreciate your worry at the moment.

I agree with Dancinggirl about oestrogen. I am on full HRT and this was on advice to my GP from the gynae and my pelvic floor specialist nurse that full HRT was needed to prevent future prolapse so you may wish to investigate this further. Vagifem, though helpful, was not strong enough to keep the pelvic floor tissues strong in my case.

As for the air bubble feeling. I think you should get some medical advice on this to make sure that you haven't developed a fistula which can cause an air-bubble sensation. Is the air expelled when you move or stretch?

There may also be support and advice on the hysterectomy UK site?

Let us know how you get on. There are a few prolapsed women on here so I'm sure you'll get lots of support.

Thanks for the lovely welcome taz. I actually wondered about a fistula . I'm not sure how they are diagnosed. I think I will see about it. I would love to hear from someone who has had one. I don't have the symptoms of bowel contents coming through vagina, I know that can happen after diverticulitis if fistula is between bowel and vagina, but there are other places they can occur, and I have wondered about the air and extreme discomfort of the bladder. One thing I have learned is that everyone is different and I could have a fistula with unusual symptoms. I think the doctors/specialists always go for the obvious symptoms and they aren't always that obvious or common. This is the forum to find out . It's awesome. Thank you.

Glad to be of help. I did find a site about prolapse and fistulas. You can get one between the vagina and urethra if I remember rightly which can cause bladder discomfort. I'll try and find it later.

Scottishgirl - I doubt very much that the Vagifem would have caused or irritated the Divericulitis - if anything it would help to maintain a healthier lower bowel.  Diverticula occur because the wall of the lower bowel weakens, pockets appear in the bowel wall and these pockets can become infected.  The leaflet I was given talked about constipation or lack of fibre being a possible cause but I'm a vegetarian with a high fibre diet and open my bowels at least once a day very easily, often twice or three times!!!! The my Diverticulitis emerged, I had stopped HRT a year before and I had been on PPI drugs which can increase bowel movements, so the lack of oestrogen plus the PPIs were more likely to blame. I will not take PPIs again - or at least not for any length of time, as they really mess up the digestive system and are known for causing lower bowel problems. 
I will be using local oestrogen (Vagifem) for the rest of my life though and to prevent ongoing problems, I follow my brother-in-laws advice (he is an eminent gastroenterologist) and eating small meals often - simple yet nutritious and drink lots of water.
Oestrogen deficiency reduces collagen which supports our skin and muscles - our digestive tract is all muscle, so therefore we are more likely to get diverticula, prolapses etc.  AS you have had a hysterectomy, you only need oestrogen and I would suggest you ask to try Oestrogel or oestrogen patches alongside the Vagifem to help maintain  the health of your entire lower abdomen - including you bowols. You may only need a very low dose.  Unfortunately I still have my uterus so find the side effects of progesterone in combined HRT too problematic now ( I am 62) or I would still be using systemic HRT. 
 Oestrogen only HRT carries very few risks - it's the combined HRT that is slightly more risky. - so you have everything to gain and nothing to loose.  Do get yourself checked out though. DG x

Thanks for that link. It's really interesting. I think I would probably have more symptoms with a fistula, but with all my surgeries, I might get it checked out anyway. Thanks so much for that .
