Premarin 1.25/Premarin 2.5mg and bleeding

Hi all

I have been on Premarin 1.25mg and Provera 2.5mg continuously for 3 months now with absolutely no problems at all.

However on Wednesday my nipples were quite sore and then on Thursday I started bleeding and now here we are two days later and I'm still bleeding with sore nipples.

I can't get into the doctors until Monday but just wondered if my body has got used to the low does or Provera and it needs increasing to a higher dose.

Or is it something else?  Any ideas, anyone.

Erratic and break through bleeding are common with any conti HRT regime in the first 6 months. If it doesn't subside and stop within the next few weeks then you will need to have this investigated but it should stop by 6 months. You may need a higher dose of progesterone to prevent break through bleeding as the balance of oestrogen to progesterone may not be right.   I would suggest you get a Mirena fitted as this is often less hassle and gives fewer side effects and problems overall. Hi squeeze

Provera is prescribed on a continuous basis at either 2.5 or 5 mgs per day as you haven't had any problems until now I would be inclined to increase the dose to 5mgs daily and see if this solves the breakthrough bleeding problem but speak to your doctor first.

Thanks to you both for replying, I did think I might need to increase the dose.

I've never had problems with breakthrough bleeding before until I came off Prempak C, I've had nothing but problems since, until having the Premarin and Provera, so will go back to the doctor and see what they suggest.

Where are you in menpopause - or rather where were you before starting HRT? If you were still peri-menopausal (when you were on Prempak C) you were on the right type of HRT to give a withdrawal bleed, but you were changed to a continuous combined regime which is for women who are post-menopausal. If you are not post-menopausal then your own cycle will still be going on and would account for your bleeding - ie you could be on the wrong regime, esepcailly as you experienced pms breast symptoms.

In your position I would not increase the progestogen dose if you think you are peri-menopausal still - and change to a cyclical regime when you would get a monthly withdrawal bleed. This would entail taking the Provera at a higher dose (10 mg) for 14 days per month. 

Sorry I can't remember how old you are but it's the stage rather than age which is more important in determining menopausal status.

Hi hurdity

I have been without a period for over 12 months now, have been on hrt for over 2 years.

Went back to the doctor two weeks ago after being on the Premarin 1.25 and 2.5 of the provera for 3 months as I had two periods, she put me up to 5 mg of provera.

So had a fab week after starting the higher does, bleeding dried up, headache went and the intense itch I have went.  Only for today it's all come flooding back again.

I just don't know what to do now and what my choices will be.

It is early days and some bleeding is normal on any conti HRT for up to 6 months - it should settle. You could Have a Mirena fitted. Easiest non bleed HRT combo is the Mirena with seperate oestrogen either as patch or gel. DG x

Thanks for responding.

I have had a mirena coil fitted in the past but didn't work for me unfortunately.

I was wondering if I could get Norgestrel separately instead of provera as this is month 5 on them now, just recently increased dose and I'm still bleeding and I have patches of red scaly skin that itch like hell as well.

I was wondering if anybody knew of any menopause specialists in the North West as I really need to get all this sorted.
