Arghh, still no bleed on utrogestan!!
« on: November 15, 2017, 05:42:19 PM »

Hoping someone can help me with some advice.  I finished my new increased utrogestan dose (200mg for 12 days) three days ago and I've not had a bleed.  Just some pain and discomfort around my ovaries.  Is this normal??  I thought the increased dose was supposed ensure there was a bleed?  and shouldn't this normally have happened by now.  Feeling worried about my womb and ovaries and whether or not the aching in the lower abdomen is normal if there's no bleed to go with it??

Ah ok Cassie, thank you. I hadn't realised that.  The only time I've had a bleed it happened during the 12 days while I was still taking the utro so I thought it should've happened by now. It's worrying not getting a bleed.

Yes, on any HRT regime, the bleed should happen 2-3 days after finishing the progesterone phase. DG x

Hmmm, more pain today and still no bleed.  Two months until my follow up at meno clinic.  Might need to see GP I think.

Do you use it vaginally.   I use 100mg vaginally for 7 days and always get a bleed.  When I took it orally I didn't always get one.  bleed usually happens two days after.  If my last utrogestan is used Saturday night then bleed usually Tuesday morning

becca - if you are getting cramps and feel as if a bleed is due, then you will probably bleed but only if there is a lining to shed. Maybe your womb lining simply hasn't built up enough to bleed OR you aren't absorbing the Utrogestan properly.
I wouldn't worry too much as not every women bleeds on sequential HRT. DG x

That's interesting Meeka - I have only ever been given the option of orally so far but am realising from reading on this forum that vaginally is an option.  I started on 100mg for 7 days and then when I didn't get a bleed for 10 months and found out that I was only on 25% of the recommended safe dose I asked my GP to refer to a specialist clinic.  Meanwhile I increased my dose to 100mg for 12 days and got a bleed on day 7 of taking it on the very first month.  So I thought I'd cracked it, but then no bleeds for  3 months on the same dose and now none on the increased 200mg dose.  I have read on the utrogestan leaflet that it can cause ammenorhea so I wonder if that is what is happening for me.  I will have to wait for my next meno clinic appointment probably to try vaginal use, unless my GP is willing to help me out.

Thanks DG that's reassuring from a safety point of view - when I did get the one bleed I have had I felt SOOOOOOO much better, it was like I was the person I used to be so I had been hoping that this increased dose would give me that too.  I got a bit tearful as I wrote that last bit so maybe, just maybe there is a little PMS and a bleed on the way.  ;D

I use 100mg utrogestan vaginally every other day specifically not to have a bleed.

How much oestrogen are you taking as 200mg is a heavy dose and if you are using little oestrogen there may well not be enough lining to shed.

Out of interrest, are you on a continuous HRT regime then Lizzieb60?  Or just prefer not to have a bleed?

Hi Cassie,
I started on 3 pumps of oestrogel initially, then had to reduce it to two pumps as I was getting anxiety and depression symptoms.  2 pumps  worked great for me for about a year.  But increased it to three pumps again three weeks ago as some hot flush and night sweat symptoms had returned - they disappeared as soon as I increased to 3 pumps   My understanding was that 200mg utro was the standard dose and that less is only if you are progesterone intolerant and under medical supervision??  I have recently been referred to an NHS meno clinic and they have advised me to increase to the standard dose for three months to see how it goes.
I am wondering if I need the additional oestro now due to more progesterone? would rather be taking less of everything!

becca - stick with what you have been advised by the clinic.  We are all different, so respond differently to the various HRT types and regimes.  Am I right in thinking you are still quite young?
There would actually be more concern if you were getting erratic or heavy bleeding.  If the clinic was concerned, then they would have you scanned. Try to relax and get on with life - many of us would be thrilled if we didn't bleed on sequi HRT!!!  DG x

Good advice DG, yes I'm quite young 46 borderline early meno - was still having bleeds at 43 but up to 100 days apart.  The sun is shining today so I'm going to get my work down and then some exercise!  Thanks for listening and for the advice x
