Smear Test - Pain

Good morning all.
Last week I went for a routine smear test.
It could not be done. Couldn't even get the speculum in, it felt like a knife.
The poor nurse made me an appointment for a few weeks time and said to take paracetomol beforehand.

I've never had a problem before. I am 51 and most of the menopause stuff has passed by.
Last year I had a couple of small periods and discovered I could no longer use tampons unless they had a plastic applicator.
Couldn't get them in as they hurt.

The nurse said if we couldn't do it I might have to have it under general anaesthetic.

I am scared stiff of this. A general isn't going to make me more 'elastic' and they will rip me apart.
I have started using Replens just in case it is atrophy.

Also, although single for 3 years (too may idiots out there...) I am a tad scared that is also all over if no one can get near me.

Has anyone else suddenly had a problem with that speculum thingy?

I think you may need to visit your GP and ask about atrophy and Vagifem.
After using that for a while then things can get much easier.

I had problems too and the Vagifem helped no end along with the smallest speculum that they had.

I'm sure others will be along with good advice but I just wanted to say you are not alone in this and things can be done that will help. I would think that a,GA would be the last resort to be honest.

Oh dear ....paracetamol before hand 🙄 You need local oestrogen, your vagina is atrophying due to VA , vaginal atrophy .

I no of a lady who had to use vagifem every night for four months , to be able to do a smear test and that was with the virgin speculum.

Some serious education needs doing amongst these health care professionals.

BTW Replens is no longer recommended, just hasn't worked its way down to general knowledge, all to do with osmosity and PH levels .

Thank you all so much for your quick replies.

I will speak to the doc about that.
A few years ago I was given some local oestrogen cream, Ov...something or other, and I reacted badly to it. Couldn't have HRT either.
If it hurts that much again, I probably wont bother going back. Yes, I know... :( so I will make the effort.

I could be very wrong and this could be totally the wrong thing to do but I'm sure I read on the forum many years ago some women used something,and I really don't know what, to gently and slowly stretch things.

Maybe someone else would be able to comment on that but of course you would need to speak to your GP first.

Hello Biscuit, as the other ladies have said, Vagifem and/or a local oestrogen cream should help you no end & this sounds a very sensible request to make to your GP.  Some ladies prefer the cream for internal use, some Vagifem, which is a tiny tablet with very slim applicator.  I know you had problems with the cream, but there are 2 strengths & these can be used externally in addition to Vagifem if needed.  I was prescribed the stronger, Ovestin & wonder if this is what you also had which may have been too strong for you. 

I had no probs with smears until about 10 years pre-menopause, then found they became gradually more painful.  At the time, I had no idea why & put this down to a tendency to persistent thrush & residual rawness from long periods that always left internal skin sore.  Then post-meno, but before starting Vagifem, had a shocking smear that was all but impossible - so painful it took ages, even with the tiny speculum.  I was shaking & heart pounding & the poor nurse I knew well & was relaxed with, was red in the face with the stress of it! 

I hadn't heard of VA until I found MM, but after starting Vagifem (& now together with full HRT), gynae exams & procedures (inc a recent smear) became much easier.  I hope you manage to get some topical treatment prescribed easily and will soon feel the benefit.  If you are worried about the consequences of atrophy on possible future relationships as your post suggests, I would also mention this to your GP, so that she can refer you for specialist advice if need be.  I think Mrs Brown may be thinking of dilators, but have no personal experience of these.  I went 2.5 years post-meno before starting Vagifem & 3 years before starting full HRT, but with these meds sex has become increasingly comfortable in the following 18 months, so please don't assume it's too late & give up hope of feeling better.  Unfortunately, Vagifem won't do anything to reduce the number of idiots out there.   ;)
