Hello again Mrs Brown, was thinking about you in the night! I hope this is not too personal, but you mentioned a bowel prolapse which makes me think you suspect this may be connected to the problem with bladder accidents when the urge strikes. Not sure whether only your GP has diagnosed the prolapse, or if perhaps you have also been seen by a Gastroenterologist. If not, I wonder whether this might be worth considering as an alternative first port of call? If the prolapse makes emptying the bowel difficult and/or there is a tendency to constipation causing heavy bloating, you will probably realise this may be putting pressure on the bladder and adding to strain on the pelvic floor. If you have noticed the accidents tend to occur when perhaps your bowel is fuller than you'd like, I wonder whether help with improving any bowel dysfunction might be valuable for the bladder control issue. I certainly find that pelvic floor exercises are more difficult with a congested tummy. In case you don't already know this, some Gastroenterology depts have consultants with a special interest in pelvic floor disorders and can offer investigations to see how well the relevant muscles function, treatment to improve bowel function or referral for pelvic floor physio if this is indicated. I would hope your GP will know if this is the case in your area.
Hopefully resuming Vagifem alone will solve the problem and you yourself will know best what course of action feels right at this stage, but I wish you well with whatever you choose to do.
