vaginal atrophy and splitting skin

I have successfully used Vagifem for 6 years and will continue to use it.  Sometimes I also use estrol cream externally and everything has been very stable.  Last weekend I was a bit clumsy with the pessary and the skin at the edge of the vaginal opening split near to my episiotomy scar and I nearly hit the roof!  I felt like I had given birth to my son all over again and ended up sitting in a salt bath which took me back over 30 years  :(

With the aid of Sudocrem and coconut oil and wearing a skirt with no knickers (in the house  ::)) it has now recovered but is still very tender and fragile.  I have made an appointment for a review of my VA with my female GP next week.

My question is this - is it common for VA to continue getting worse even with Vagifem use.  Although I realise it does not cure it I thought that maybe it stabilised it but clearly the skin has continued to become thinner and more fragile.

How many times a week do you use the Vagifem and the cream?

Hello Tinkerbell, I use Vagifem every other night and the cream externally once or twice a week.  Everything was fine until I caught the area and pulled the skin.  I was surprised how thin and unstretchy it was.  Hard to imagine two babies heads have been through it!!

I get little splits in the skin and use Ovestin cream on the outer areas five times a week, I also use Vagifem and systemic HRT to cope with the VA.
Maybe try the cream more frequently also have you done a Vagifem reload recently?

Yes I am using it every night until I see my doctor next week.  I find Ovestin burns terribly but now the split is healed i will try using the estriol cream more often.  I find coconut oil very soothing, I read about it on here, but it doesn't heal the skin once it is split  :)

Generic estriol cream is one tenth the strength of ovestin and maybe better to use liberally on the external areas (ie with less burning maybe?). You would need to check the excipients from the product info to see how they differ.

Thank you for the suggestion.  The comparison is scary, the fact that chlorhexidine is put into ovestin no wonder it burns!  :o

Estriol cream contains 0.01% estriol. The other ingredients are benzoic acid (E210), glyceryl monostearate, arachis oil (peanut oil), glycerine, glutamic acid, purified water.

Each 0.5 g of Ovestin Cream contains 0.5 mg of oestriol. Inactive ingredients: octyldodecanol, cetyl esters wax, glycerol, cetyl alcohol, stearyl alcohol, polysorbate 60, sorbitan monostearate, lactic acid, chlorhexidine hydrochloride, sodium hydroxide and purified water.

I had: have this .

I saw Nick Panay last week and he could see the episiotomy scars were thinning.

So for me vagifem 7xs a week and ovestin 4 Xs a week.

He said with a dilator , put a good blob on the end ( size of thumb nail ) and gently massage it into the scar , this should help the scar tissue become more supple over time . He said this is still a small dose of local HRT.

However for me I also need 1.5mg of sandrena also, he increased it by .5mg and it seems to be helping , the perinium appears/feels more stretchy.

I look like a patchwork quilt down there , and a year ago the scars were on the brink of splitting and I was told by some specialists , it looked fine for my age ☹️️ hence my loss of faith in a lot of specialists.

I took photos at the time ( wish I had kept them ) because the before and after are amazing.

I did 18 months of local HRT with very little help, so as Dr Annie said some of us need the lot regards collegan.

Did you have stretch marks ? I have dreadful ones and I wonder if that's part of it ?

Ovestin used to burn me also , but now I am topped up it doesn't .

Find the whole thing terrifying TBH, regards old age ☹️️ Who will look after our bits then ?

Yes I wonder how three 9lb babies came out of my poor vagina. 😱

Sorry to hear of your woes BabyJane it's a constant battle with this VA

I use Hydromol and the dr gave me some Yes vaginal cream to try - I put some on every morning and night as well as my  usual routine I,e Vagifem and Ovestin

Mary Jane - I too find this whole VA thing terrifying too it takes over your life   - I was just thinking the other day what will happen when I'm old and my husband said  - why worry about what's going to happen in the future just enjoy what you have now - so I'm gonna try !

That's what worries me...being in a nursing home with dementia and awful burning atrophy and not being able to communicate with the staff about your discomfort :(
