Confused by the Peri, any ideas?

I recently have completed 3 months on evorel sequi which I am pleased with myself for completing.

I always bled early on the combi patch and by the 3rd month, I was bleeding whilst still on the oestrogen phase and it continued for 17 days. My female gyny duly banned me from any more HRT due to my irregular early bleeding until I had a endometrial biopsy and Hysteroscopy.

Fast forward to last Thursday 15th, both procedures were done under a GA as I am such a lightweight as this type of thing.  Anyway multiple small fibroids and another larger one in the lining of my uterus. According to the gyny my womb is quite nobbly! Therefore I have to return in 6 weeks time to discuss an ablation. 

Yesterday I seem to have started a period, the loss is far too heavy to be connected to the operation. So can I deduce that I now must be making some of my own oestrogen and progesterone again? I have noticed fewer flushes of late, sleeping well, and normal mood.

Now I'm thinking that the long period I had at the end of month 3 on evorel, some of it was withdrawal bleed and some of it was my own. As approx 34 days later, my own period has arrived.

Can HRT kickstart your own hormones or is this just what to expect between now and post menopause.

I'm not starting HRT due to the impending operation but when it's over, which is the only way to override my own cycle? Should I try a pill as I am 45? Otherwise surely, this could continue to happen until I get to post?

I know there's no quick or easy answer but just wanted to moan and vent! What a total nightmare?

Sorry to hear about all your bleeding. Unfortunately you cannot say how your body will behave hormonally until you reach menopause and of course if you have fibroids then extra heavy bleeding can sometimes occur during parts of the cycle. If you stopped HRT at any point during or just after the combi patches then you would expect a withdrawal bleed as part of the HRT  and the progestogen acting on the womb lining. If you have gone a whole month off HRT and still getting a bleed it is likely to be either due to over-thickened lining (anovulatory bleed) which does not necessarily occur at the "right" time, or the firboids (?) or as a result of your own ovulation.

Yes you can override your cycle via the Pill - the most recommended at your stage ( per-menopause) being the ones with estradiol ie Qlaira or less commonly prescribed - Zoely. Some women override their cycle through high doses of oestrogen ( prescribed through specialist gynae) without the extra high doses of progestogens found in the Pill.

Many thanks for your kind and knowledgeable reply.

I came off HRT at the oestrogen only phase so never made it to the progesterone phase.  The heavy bleeding again has caused my ferritin levels to plummet yet again just over a year after the last time. I'm taking my iron tablets 3 times a day as prescribed.

I think you are right, the bleeding problems complicate the peri. Therefore I really need to resolve the bleeding, then sort out a HRT/Pill regime for me. Qlaria was my main option but I will research Zoely too.

On the whole, I'm doing really well and have come on loads over the past couple of months, but with the heavy bleeding, always comes the mood drop and depression. This time last year, I couldn't have had an operation and now I have, so I am advancing in the right direction.
