HRT dosage in perimenopause

Hi Ladies,

Just wanting to get your insight about something:

I've read many forum posts about HRT and I have adopted the strategy: 'start low and go slow'. I am on 25 mcg estradot patches and 100mg utrogestan for 15 days. I started the HRT 12 days ago and I am perimenopausal.

I'm a little confused about dosages: Should a person in perimenopause need a lower dose of estrogen than a person who is post menopausal? Or do we actually need to take HIGHER doses in order to override our natural estrogen production and therefore stop the fluctuations that cause our symptoms?

Any advice is appreciated

Thought provoking question Soniad!

As far as I know your progesterone will be low in peri with ip and down estrogen fluctuations. Appsrently by the time your periods stop your progesterone from ovaries has bottomed out... Will be good to hear what the other ladies experiences were.

There really isn't a "one size fits all" thing with HRT. Basically you take the dose that helps you manage the symptoms you have. While doctors will generally "start low" as you said, they will also go by your symptoms rather than either blood or saliva tests or where you are on the menopause scale - at least the ones who know their stuff about menopause that is. ;)

Thank you Matilda and Dana for your replies.

My sister is a doctor and says that the goals is to use high enough a dose so as to override the existing ovarian function. But I'm not convinced that higher is necessarily better.

Dana, that is why I started low. I'm going to give it 6 weeks and see how I feel.

As Dana says each individual is different,you need to find what suits you most of the time. Being peri that can be difficult because of hormone surges ,that said when I was peri did great for 5 years on the same one dose. Good luck with finding yours.

I believe it is very difficult in the peri stage generally.  Some women need very high doses to override everything but in my peri stage I had just one pump of oestrogel daily (so a low dose) which suited me 'just right'.  When I tried higher doses I then got headaches and other side effects. Starting low is always a good strategy with anything and in early peri meno I think using a small dosage can be enough to just balance things out a bit.
As others have said there is no one size fits all. Suppressing ovulation is quite another thing - if your low mood etc is strictly to do with the fluctuations - so comes and goes - then I think you can have something that actually shuts down your ovulation completely and then add in the hormones you need in a controlled way - this needs to be under supervision of a specialist gynae.

If I remember rightly, it's your quality of sleep that is your main men symptom and I'm afraid that whilst HRT can improve this, unfortunately many of us find good nights sleep more difficult to achieve as we get older.

There is a great danger of ‘over thinking' everything when it comes to HRT - so many women want perfection and are constantly looking to find something that will make them feel good every day - HRT treatment takes time and a bit of experimentation but each dosage you try needs at least 3 months to settle and do it's magic before switching HRT or changing doses.
I'm afraid patience is the key - and don't expect anything to give results for at least 4-6 weeks. Dg xxx

I plan to stay on this dose for 6 weeks and re-assess from there.

I started.low but now on four pumps of oestrogen a day to suppress ovulation.  Its not really helping.  Considering the pill. It has stopped my periods but that's all

Tinkerbellj - I think your experience illustrates who different we all are.  Suppressing ovulation isn't always the answer and in the peri stage it is very difficult to get the balance right.
There is that contraceptive pill that has bio identical oestrogen which many women in peri find good - can't exactly remember what it is called - Quaria or something - someone will let you know I'm sure. DG x

Hey dancinggirl gow r u doing?  Yeah its qulara I habe started ads as well amd am gettimg tests for b12 deficiency.sortt of hoping I do habe pernicious anaemia cuz at least it can be treated and an answer to the dreadful fatigue.  R u still on hrt? Hope u r feeling well x

Hi tinkerbellj - I'm still only using local oestrogen (Vagifem) but no systemic HRT.  In the hot and humid weather it has been really difficult to control my body temperature - I find I sweat a lot in the head and face when not on HRT - but apart from poor sleep I'm doing OK.  Now 60, I really thinking my body is struggling with HRT - it simply doesn't want to adjust any more.  Whether I go back on HRT or not only time will tell but at the moment it's actually a relief not to have all the problematic bleeds and cramps.  I always say - 'HRT has to bring more benefits than side effects.'
I am making myself rest a lot more which enables me to cope better.  DG x
