The mediflow pillow has water put in to which ever level you choose. Firm medium or soft. I chose soft as I would normally use a feather pillow and like my head to sink in rather that lie on top. My neck has improved so much that I have been able to stop taking naproxen twice daily. There has been a bit of a flare up over the past 2 days due to having the flu. Overall though it was well worth the money. I bought it from Amazon x
