Hi Amandajane

How long have you had the Mirena and the wrong Evorel (conti)? The excess progestogen could be affecting you and making you feel fatigued, especially on top of your own cycle, if you're still having one. Sometimes the synthetic progestogens have the opposite effect though - the side effects of norethisterone are fatigue or insomnia! You also may be experiencing side effects from the limited systemic absoprtion of the progestogen in the Mirena coil too..... Hopefully you will feel less fatigued when you get onto the right patch.

I do think Hurdity is right and that you on far too much progesterone.
I would get back to you doctor and get this changed.  If you are getting on OK with the patches then the Evorel with oestrogen only would be good or perhaps try Oestogel if you don't mind applying it daily. I suspect it was an error that you got the conti version of Evorel by the doctor when making out the prescription!!!!

Hi again

It could be that the doctor deliberately gave you the Evorel conti because of the bleeding you were still experiencing on the Mirena - however it looks like you were given the Evorel conti after only a couple of months on the coil and it can take much longer than this to settle. However I can see how debilitating that could be. This is what it says under the Summary of Product Characteristics:

Bleeding irregularities

Irregular bleeding: Mirena usually achieves a significant reduction in menstrual blood loss in 3 to 6 months of treatment. Increased menstrual flow or unexpected bleeding may be indicative of expulsion. If menorrhagia persists then the woman should be re-examined. An assessment of the uterine cavity should be performed using ultrasound scan. An endometrial biopsy should also be considered.

Has the bleeding stopped now and were you on oestrogen only patches with the Mirena before, or no extra oestrogen at all?

If so I would ask for Evorel only patches and see how you feel/what happens and if you feel less fatigued. You might eventually benefit from a higher dose of oestrogen depending on your age/stage of menopause.

It's horrid, isn't it?  >:(. Does yours last all day or is it intermittent? I can feel fine one moment, then literally unable to hold open my eyes an hour later. For me, I think it's aggravated by lack of progesterone, believe it or not! I always get it when I am not taking progesterone, usually during and just after my period. I have come to dread the end of my period and the days after - something that seems to be contrary to everything I read. So confusing, isn't it! X

Amandajane46
I had a Mirena for over 4 years and I now realise it did make me feel slightly sedated sometimes.  I would get these bouts of complete exhaustion and I always put it down to being tired from working so hard or being under stress.  I am now on a different HRT using the bio identical progesterone Utrogestan in a sequential form (so not taking it all the time)and I'm not getting this type of fatigue.
I don't know whether you have told us why you were given the Mirena? Were you having excessive bleeding or were you simply getting bad flushes and night sweats? I am still baffled as to why your doctor has given you the Mirena as well as a combined continuous HRT including both oestrogen and progesterone!!??
You say you just get really tired around 2pm in the afternoon and around the time of your period? This is quite common - particularly when we become peri menopausal.  Many people get a drop in energy after lunch and want to have a sleep, especially if you have had a lot of carbohydrates.  If you have mainly protein at lunch this can help reduce this drop in energy.
Your doctor was right that your would need some form of progesterone to protect your womb lining from  thickening but having fitted the Mirena (which also gives you contraceptive protection) you then only needed to add the some oestrogen in patch of gel form. I still think you need to check with your doctor whether they have given you the right type of Evorel patches. Also why give you a conti patch when you are still peri menopausal - this might explain why you get random breakthrough bleeding as well!!! You need to go back to the GP and ask why they have given you the extra progesterone in the combined patch you are using when the Mirena should have been enough.
Good luck DG x

Just replying to this as I'm exhausted - I just don't sleep - I used to - but last few months in particular have been awful.  My eyes are so puffy that people keep asking if I've been crying - I can literally see the bags on my cheeks.  Got my appt tomorrow with my lovely GP so hoping to get a few things sorted out - change of HRT and something for the coccyx - then maybe, just maybe I can get some sleep.  I know there's no answers, but sometimes just helps to get it off your chest doesn't it!
